Job summary
As a specialist Nurse Dementia Therapist in this setting, you will be responsible for providing the highest level of personal care and attention to people living with dementia, formulating, implementing, and maintaining care plans. You will also supervise the staff in the Reconnect community, ensuring the delivery of care is within the expectations of the Reconnect model.
You will ensure that people are treated with respect and dignity, your approach is empathetic, compassionate and focused on ensuring their life is the best it can be, focusing on the strengths and abilities of each person.You will need to be understanding and patient towards people and their needs, and encourage, assist and support people to participate in appropriate leisure activities and activities of daily living, normalising life as much as possible with joy and happiness.
People living with dementia often have continuing periods of loss and distress which inhibits them from living life to the full in care homes. Orchard Care Homes are aiming to transform and reshape care for people living with dementia in our homes. Main duties of the job
To assist with the formulation and delivery of the individuals care plans. Work as an integral part of the caring team, to deliver the highest standards of person centred care, to people who are living with a complex dementia. Adopt a holistic approach which encompasses the needs of both the person and their families to ensure that at they retain their identity, are comfortable and are engaged in meaningful activities. To ensure that communication is appropriately facilitated, that people regain and retain their independence, their choices are met at all times and that they are treated with respect,dignity and given their rights to privacy.
To administer medication and liaise with external professionals and ensure that people receive the appropriate referrals and medical attention.

Providing
leadership to the staff team which develops practices and ensures company
policies and procedures are followed in full.
To
support residents based on individual ability with all aspects of personal care
including toileting, bathing etc. following individual care plans carefully and
ensuring that all contact is polite, friendly, warm and supportive. To act as a
key worker for a named group of residents, ensuring that care is delivered in
line with residents personal needs and preferences.
To
provide and supervise the delivery of nursing care, acting as a resource for
all the caring team, identifying any training and development needs, supporting
colleagues to fulfil their potential and assist in training programmes.
To
dispense and administer medication to residents as prescribed, accurately
maintaining appropriate records in line with current legislative guidelines.
To
carry out effective stock control and ordering of residents prescribed
medication and medical products for the home in line with current legislative
guidelines.
To
maintain NMC professional registration and be conversant with the NMC Codes
including; professional conduct, scope of professional practice, standards of
administration of medicine, standards for records and record keeping and
exercising accountability.
To
understand and comply with all statutory and legal requirements which are
relevant such as Health & Safety, COSHH, all aspects of the Care Standards
Act, have a clear understanding of Infection Prevention and Control policies,
procedures and measures and ensure that the correct accident and incident
reporting procedures are followed to maintain a safe environment throughout the
home.
To
