Job summary

We are seeking a highly motivated and dedicated Clinical Fellow to join our dynamic General Surgery team. This post offers an exciting opportunity to develop clinical skills across both elective and emergency surgery. The fellow will be mainly based in emergency general surgery (EGS) but will be expected to also spend time within elective commitments, gaining a broad experience in managing a variety of surgical conditions.

This post does not have an on-call commitment and the hours of work are Monday to Friday only with daily hours of 8am until 5.30pm. The successful candidate will have undertaken at least 4 months of general surgery in their foundation programme. The candidate will rotate between elective and emergency services on a fortnightly basis. Main duties of the job

The successful candidate will be able to combine service commitment with significant professional development in emergency and elective general surgery. This is an ideal post to gain experience towards applying for core surgical training or to add to core surgical training prior to application for higher surgical training.

To support the post holders continuous professional development and career aspirations, Clinical Fellows will be supported by Consultant Clinical supervisors.

Key Responsibilities:

Emergency General Surgery (EGS) Service

Play an integral role in the emergency surgical service, supporting the Foundation doctors in the management of inpatients and acute admissions.

Assess and manage patients admitted through the emergency department or referred for urgent surgical review.

Provide support and leadership within the Surgical Same-Day Emergency Care (SDEC) service, managing patients returning for ongoing assessment and treatment.

Assist in emergency theatre, with opportunities to advance skills in:

o

Abscess drainage

o

Skin suturing

o

Laparoscopy (including appendicectomy)

o

Emergency laparotomy, with advancing assistance skills under supervision

Participate in the management of acute surgical conditions, including trauma, bowel obstruction, perforated viscera, and other common emergency surgical presentations.

Attend and contribute to daily emergency surgical team rounds and ward rounds, ensuring optimal patient care and timely management.

Support and supervise junior medical staff, particularly FY1s, in the care of emergency surgical patients.

Other surgical experience

The post holder will have a session of personal development time (PDT) each week which they can use to gain further experience across the surgical services, planned in advance.

This can include other surgical specialties such as urology, ENT or trauma and orthopaedics.

Although the first priority for the post holder is with regards to EGS responsibilities, it will be expected that the post holder will be proactive in gaining clinical experience on a day to day basis when they are not busy with EGS activities.

For example, attending theatres, clinics or endoscopy lists.

The post holder may be asked to assist in elective lists as needed.

Audit and Quality Improvement:

Actively participate in ongoing audit and service development, particularly within the EGS service.

Work with senior clinicians and management to identify areas for improvement in the emergency surgery pathway and implement changes based on up to date evidence and best practice.

Lead and participate in audits aimed at improving the quality of care within the emergency general surgery service.

Use these audits to contribute to the development and implementation of new pathways, protocols and service models, to enhance the efficiency and safety of the emergency surgical services.

Maintain an up to date morbidity and mortality record and present these patients at the morbidity and mortality meetings.

Training and Education:

Participate in regular clinical teaching sessions, audits, and departmental teaching rounds.

Attend designated clinical fellow teaching sessions.

Attend speciality MDTs and radiology teaching sessions.

PDT time will be condensed into elective weeks and can be used to attend clinics or operating lists in other surgical specialities.

Attend departmental educational events, including operative and non-operative learning opportunities.

Receive dedicated supervision from senior surgical consultants and registrars to enhance surgical knowledge and skills, with a focus on developing proficiency in emergency and elective general surgery.

Utilise simulation training to enhance surgical skills including the laparoscopic trainer.

Flight transfer

Jersey is aligned to the UK health system and relies on certain centres for tertiary care. Flight transfers are provided by a hospital based Jersey Emergency Transfer team (JETs).

Patients being transferred acutely to tertiary centres such as the Cardiothoracic Unit in Oxford and to the Neurosurgical Unit in Southampton for further specialist treatment are accompanied by a doctor and a trained nurse.

Appointed Clinical Fellows will be expected to be trained to carry out flight transfers. Participation in transfers that take place out of hours will be on a voluntary basis and will command additional remuneration.

Additional Professional Development

Medical students from Southampton University are attached for 8 week rotations in their final year. Clinical Fellow post holders have enjoyed interaction with these enthusiastic trainees and have provided bedside teaching in addition to that provided by Consultants and Middle Grade Doctors. Clinical Fellows have organised mock OSCE in the 7th and 8th weeks of their attachment in preparation for their final examinations. This has been a mutually beneficial exercise.

The Education Centre is a shared facility for Health and Care Jersey and is equipped with a newly built Clinical Skills Lab which has assisted Clinical Fellows in self learning, particularly in procedural work and optimising their cardio-respiratory resuscitation skills, and in teaching their more junior colleagues both medical students and nursing staff.

There are designated clinical fellow tutors and teaching programme which is in protected time which the post holder will be expected to attend.

The duties of the post holder are expected to be flexibly worked, as inevitably a number of activities run concurrently. Although there is no on call commitment with this post, weekend locum work may be available.

The Surgical Service

Given Jerseys size, the Department of General Surgery provides a very comprehensive General Surgical Service. The department currently runs with 5 consultant general surgeons each with subspecialty interests: Emergency surgery, Colorectal, Upper GI, Vascular and Breast surgery.

In addition, there is a full time consultant urologist, Mr Ben Hughes, running the urology service.

A wide range of general surgical subspecialty operations are offered on island with a small minority needing transfer to the UK for more specialist treatment.

HOSPITAL DEPARTMENTS

In colorectal, Mr Miklos Kassai offers minimally invasive service for colorectal malignancies and full service for all other colorectal conditions and proctology. There are 80-90 major colorectal resections a year for cancer and other benign conditions.

In upper GI, Mr Jon Shenfine, provides a benign upper GI service including bariatric surgery.

In breast, Mr Matthew Stephenson (Clinical Lead) is an oncoplastic surgeon offering therapeutic mammoplasties, reconstruction and skin oncology.

In vascular, Mr Mark Whyman, offers a wide range of general surgical and vascular services such as peripheral vascular reconstruction and carotid endarterectomies, with aneurysms (elective and emergency) being sent to Bournemouth.

There is an active interventional radiology service working closely with the surgical department. A wide range of interventional vascular procedures are provided on island.

The Emergency General Surgery (EGS) service is lead by Miss Rebecca Fallaize who, along with Mr Muhammed Aleem, provide a consultant led and delivered emergency service. There is a newly opened surgical Same Day Emergency Clinic (sSDEC) which the clinical fellow is actively involved in managing. The EGS service is an area of ongoing service improvement and the postholder will be instrumental in this.
