Job summary

The post holder will provide specialist Speech and Language Therapy services to the rehabilitation ward in Jersey, working as an integral member of the rehabilitation multidisciplinary team and to provide support and mentorship for less experienced Speech and Language Therapists.

The post holder will support service development with the whole rehabilitation team and will provide outreach into the acute and community settings as required. This will include working with a range of patients from those individuals who have experienced a CVA to those with progressive illnesses and severe and complex traumatic brain injury and involve working with them towards their discharge into the community. It will involve working closely with the multidisciplinary team across rehab and community settings supporting the individuals across their rehabilitation journey.

The post holder will also work with the Memory Assessment Clinic providing detailed Speech and Language Therapy assessments to help provide information for diagnosis of different types of Dementia. Supporting and providing advice for clients and their families with Dementia and wider service development in support of the services these clients will access Video fluoroscopy skills and experience in carrying out and running FEES clinics desirable.

Good knowledge of AAC essential.

The applicant needs to be flexible and work across different areas, as small team and need to be able to support with caseloads across a range of individuals

Main duties of the job

Clinically lead, develop and provide a highly specialist Speech and Language Therapy (S<) service for the rehabilitation unit, elderly mental health and traumatic brain injury services.

To work as an autonomous practitioner implementing highly specialised assessment, evaluation, treatment and management of patients with complex needs.

Providing highly specialist professional knowledge for other disciplines so that the contribution of Speech and Language Therapy can be integrated within the total service provision of the rehabilitation ward, elderly mental health service, and people living in the community with a traumatic brain injury.

Providing advice and training to clients, their families and other professional staff on the management of complex communication and swallowing disorders.

To work as an advanced practitioner in video-fluoroscopy and FEES (Fibre Endoscopic Evaluation of Swallowing) clinics as required. Due to the nature of being a small team the post holder will be expected to support in other areas as required e.g. on the medical wards and community settings.

Job responsibilities

Manage a clinical caseload of complex clients. Developing and implementing a specialist area of speech and language therapy. Identifying needs, researching, interpreting and implementing developments in speech and language therapy practice, and contributing to broader organisational changes in order to support a continuously improving high quality service to these clients.

Disseminate, interpret, share implement and monitor Government and national policies, standards and guidelines within specialist area to ensure that work is conducted in accordance with organisational polices and best practice.

Receive and manage direct referrals from a range of professionals from in-patient and community settings, diagnosing and screening clients to identify how they may benefit from Speech and Language Therapy and providing assessment in group or individual intervention as needed with other professional disciplines, service providers, agencies and families to ensure each client receives the specific support they need.

Prescribe appropriate treatment to assist the clients to gain additional function where possible and to enable the client to maximise the use of existing function and /or to reduce anxiety and frustration by modifying the communication/physical environment to minimise the impact of disability on quality of life.

Provide information, support and advice to other professionals in differentiating their service provision to enable clients with communication impairment to access their services.

Manage cases/key working of clients with disabilities and offer support to carers to promote effective outcomes that are consistent with the clients needs, referring to other agencies as required.

Maintain appropriate patient records of assessment intervention, progress and relevant statistics and administrative records regarding all areas of work, to monitor the effectiveness of intervention, to facilitate intervention planning and long term client management and to support the ongoing evaluation of the efficiency and effectiveness of the service to inform departmental service planning.

Develop and deliver highly specialist training programmes, providing information and advice to professional qualified and unqualified staff, voluntary organisations and family carers in order to improve the understanding of clients need and promote effective management of communication and /or other disorders to minimise the impact of disability on quality of life.

Act as mentor and provide regular supervision, advice, support and training for less experienced staff, students and staff from other services to assist with client intervention, and to support them to deliver their roles to the best of their abilities and maintain their continuing professional development.

To work as an advanced practitioner in instrumental assessment of swallowing in Therapist led Videofluoroscopy clinics and FEES clinics.

To provide consultative advice to Consultants e.g. the Neurologist and Psychiatrist, to enable an accurate diagnosis, where diagnosis of communication is essential to the primary medical diagnosis e.g. primary progressive aphasia
