Job summary

This post is advertised as a new Community Paediatrics post to complement the Community Paediatric work currently undertaken by the General Paediatricians. Current consultants have developed skills in Neonatology, Diabetes, Asthma and Allergy, Epilepsy, Community Paediatrics, Oncology and Neonatal Aero-medical transport. This is an exciting opportunity to steer how Community Paediatrics is delivered in Jersey and evolve the service at a crucial stage in healthcare delivery for children on the Island.

The successful applicant will be part of a team delivering community medical care to children in Jersey. It is not envisaged that the doctor will take part in the 1:4 on call, but given the island context, there may be occasions where the post holder is asked for support with some on-call arrangements.

Main duties of the job

Management of paediatric and neonatal emergencies is not uncommon. Because of the geographical isolation on the island the post holder, if contributing to the on-call rota will need to be confident dealing with these eventualities, with support from our local ITU and intensivists. There are also close working links with tertiary Centres like Southampton general hospital. There is a training programme including simulations and APLS/NLS courses which run in Jersey to support CPD in this area.

The post holder would be required to deliver 7.5 sessions of direct clinical care (DCC) per week. There are 2.5 sessions per week of SPA time. If the post holder wishes to take part in on call duties then 1.5-2 sessions in lieu of on-call are given reducing DCC.

Duties would include general Community Paediatric clinics and DCC related to this activity plus additional roles to be decided at interview but likely to include assessment and management of children with:

  • General medical problems
  • Specific developmental problems (such as ADHD or autism) learning difficulties (if a medical or neurodevelopmental cause is suspected) complex disabilities sensory impairments such as visual difficulties or hearing loss.
  • The Community Paediatrician would not generally deal with one-off, short-term illnesses. Instead team members offer long-term support, co-ordination of services and management on a continuous basis.

The General Hospital

Jersey General Hospital is the sole location of acute hospital services in Jersey. The hospital provides approximately 250 acute beds, 2 day case theatres, 4 main theatres and 2 more being constructed. There is a dedicated endoscopy suite and Breast Clinic along with a full range of other services and facilities. A wide spectrum of care is available.

Safeguarding Medical Examinations and preparation of medical reports including attendance at Strategy discussions and ICPCs

A Job plan will be agreed on commencement to include taking on a proportion of the Community Paediatrics duties currently shared amongst the General Paediatricians.

There is a Child Development and Therapies Centre where the post holder would be based alongside the team of Paediatric Therapists and Family Partnership Workers.

We have a governance system, local policies, guidelines and pathways and audits. Safeguarding in Jersey is co-ordinated by the Safeguarding Partnership board. Participation in urgent strategy discussions and Safeguarding medical examinations is currently the responsibility of the on-call Consultant of the week, but Staff Grade doctors participate in safeguarding activity including some strategy discussions and medicals. The post holder would be expected to participate in and support these processes.

In addition to SAS doctors, we have a GP ST1 trainee. We also have medical students on attachment from the University of Southampton. Participation in teaching, training and supporting these doctors and students would be expected

The post holder will be expected to participate in the training of Medical Staff for whom he/she is responsible and ensure that training and learning needs are identified and facilitated. Junior doctors will be appraised at appropriate intervals.

The post holder will be expected to take part in management and audit activities and represent the service at Hospital meetings, as agreed with colleagues. The post holder will be expected to be fully involved in a Risk Management programme and support the Governance agenda.

There is a Consultant Locum Policy that encourages cover within the Department without employment of locums. Extra duties are financially rewarded. The policy does allow a locum to cover leave for short periods if necessary. Applicants should acquaint themselves with this policy by requesting the policy document from the HR Department.

The successful candidate should be adaptable, flexible and innovative in order to meet the ongoing challenges within the service.

We are a small department, currently composed of 5 Paediatric Consultants, 7 Paediatric Staff Grades and 1 GP trainee doctor. We will have an F2 in Paediatrics from April 2023.

We provide acute and ambulatory care to the approximately 16,000 children living in Jersey. This includes Neonatal Care, Acute Paediatric Medical Care and Ambulatory Care. Acute services are delivered at the General Hospital. Robin Ward (the Childrens ward) has 12 beds and cares for medical and surgical in-patients with 1600 admissions per year. Outpatient services are provided for approximately 850 new patients per annum. Visiting consultants deliver clinics in Paediatric Neurology, Paediatric Cardiology, Paediatric Respiratory medicine, Paediatric Gastroenterology, Paediatrics Urology and Paediatric Endocrinology.

The successful applicant will provide 7.5 episodes of direct clinical care which will include ward rounds and outpatient clinics. 2.5 Sessions per week are allocated to SPA time and there are 1.5-2 sessions off in lieu of on-call if taking part in the on-call rota. Development of a special interest will be encouraged and supported.

We receive local support from Adult Intensive Care, and the surgical team, including General Surgery, ENT, Orthopaedics and Maxillofacial surgery. We also receive support from CAMHS, Physiotherapy, OT, Speech and Language Therapy and Dietetics.

There are about 1000 deliveries in the Maternity Unit annually. We run a level 1 Neonatal Unit, but frequently need to stabilise preterm babies prior to transfer to tertiary unit. Appropriate cases are transferred to NICU by our own team, but the Southampton PICU will retrieve particularly unstable newborns. Our policy is that complicated pregnancies, including antenatally diagnosed malformations, and deliveries expected before 30 weeks gestation, deliver in tertiary units in the UK. Similarly we will occasionally need to provide urgent care to critically ill children prior to the arrival of the PICU retrieval team.

There is an active Departmental teaching programme to which all members of the team contribute. There is also a generous study leave budget and ample annual study leave allocation, to allow the post holder to maintain their CPD. Annual appraisal is mandatory, which supports GMC revalidation. We have a good record of staff achieving MRCPCH in this post, and we offer MRCPCH preparation tutorials for exam candidates. We have final year students on clinical attachments from Southampton Medical School, and F2 doctors from Wessex, so clinical teaching is an important part of the job.

All consultants have a 10 PA contract for public work which may be worked flexibly to allow private practice on and off site during the normal working week and making up time spent on public work during early mornings, evenings or weekends. Job planning is undertaken annually and PAs in excess of the basic 10PA job plan can be negotiated.

The final job plan will be agreed with the Associate Medical Director- Head of care group Women, Children and Family care and ratified by the Medical Director. The post holders job plan will be reviewed annually with the Medical Director, and will be appraised according to the current guidance.
