Main duties of the job

An exciting opportunity has arisen within Manx Care for an experienced and dedicated Social Worker to join our dynamic Integrated Hospital Social Work Team on the beautiful Isle of Man! Manx Care is working hard to change the balance of care from hospital to the community as it is known that people recover better in their homes. We aim to develop an Integrated Hospital Social Work Team that works alongside ward based Multidisciplinary Teams, as well as the Access and Capacity Team, to streamline the transfer of care and discharge pathways. This integrated team will lead discharge planning for patients with complex needs who require coordinated care and support to return home. Youll be responsible for ensuring that patients are able to access the services and resources they need to successfully transition back to their homes and communities. You'll work closely with patients, their families, and other healthcare providers to assess their needs and develop a plan for their ongoing care.
