Main duties of the job

Medicines optimisation is about ensuring that the right patients get the right choice of medication at the right time. Our goal is to help patients improve their outcomes, take their medication correctly, avoid unnecessary medicines, reduce waste and improve patient safety. You will use your working knowledge of pharmacy to work on a variety of projects within primary and community care, namely GP surgeries, care homes, community mental health and the learning disability service. Your key duties will include: Assisting the pharmacists in carrying out medication reviews for patients Collecting data for audits related to medicines and patients care Working on repeat prescribing projects to reduce waste Supporting others in the correct ordering, dispensing and storage of medicines Creating reports/letters for GP surgeries detailing work undertaken, outcomes and recommendations Our team is friendly and supportive, and we are very forward thinking with regards to developing the service. You will have a real opportunity to contribute to the plans for the future of the team.
