Job summary

The States of Guernsey are looking for a Clinical Nurse Specialist to join the Diabetes Hub.

This role is a specialist nurse caring for patients and their families who require diabetes supportive care.

Main duties of the job

This role is a specialist nurse caring for patients and their families who require diabetes supportive care. The post will work collaboratively with multi-professional teams in Primary Care, Secondary Care, in the community and in all care settings (including domiciliary visits) and applies research-based practice to achieve the optimum best clinical practice in diabetes care.The post holder will act as a clinical leader for diabetes for health care workers, patients and their carers in all clinical settings. The role also provides clinical leadership in diabetes care by using strategies such as teaching and auditing services.
