Job responsibilities
Main Duties and Responsibilities:
Fulfil the role of Clinical Lead for Sexual and Reproductive Health and HIV at the Orchard Centre.
Provide advice, screening, diagnosis and treatment of patients with Sexually Transmitted Infections (STIs), including HIV infection.
Provide contraception, including subdermal implants and intrauterine devices.
Provide and advise on PREP and PEP for HIV.
Provide consultant advice and support to multidisciplinary teams working in the Orchard Centre but providing advice to health and care professionals across the Bailiwick.
Review complex cases requiring senior input.
Manage a caseload of HIV patients, including management of seroconversion, pregnant women, starting/switching treatment and assessment and management of unwell patients.
Provide clinical advice/input for the 2 Lead HIV/Sexual Health Nurses and nursing team.
As part of the MDT to define and deliver excellent case-management practice for individuals with increased vulnerability and/or complex needs.
Work with colleagues to modernise the HIV care pathway to ensure effective use of resources.
Contribute to the wider sexual health of the community in Guernsey and Alderney by supporting and working collaboratively with other sexual health providers, including Secondary Care, Primary Care and Third Sector agencies.
Contribute to the development and implementation of the States of Guernsey Sexual Health Strategy as well as national strategies to adopt and achieve a zero HIV Stigma target by developing education and training strategies within the directorate, the States of Guernsey and across parallel agencies.
Contribute to or lead on innovations, service modernisations and development.
Work flexibly to provide remote senior medical cover.
Provide high quality care to patients.
Adhere to the Good Medical Practice standards as outlined by the GMC.
Develop and maintain the competencies required to carry out the duties required of the post.
Ensure prompt attendance at agreed direct clinical care programmed activities.
Ensure patients are involved in decisions about their care and to respond to their views.
Demonstrate, model and encourage our CARE (Compassion, Accountability, Respect, Excellence) values at all times to all patients and staff.
Research, Teaching and Training:
Act as a clinical supervisor for the nurses working within the department as they achieve Competencies in HIV and STI care.
Contribute to the departmental academic programme by providing high quality teaching and training for the various staff groups.
Ensure that mandatory training is up to date as per States of Guernsey policy.
Participate fully in continuing professional development to ensure that skills and knowledge are updated in line with relevant medical developments. (Consultants working for the States of Guernsey are allocated a training budget.)
Leadership and Teamworking:
Demonstrate appropriate leadership skills with regard to individual performance and clinical teams, also working with Third Sector organisations.
Participate in any necessary management and leadership development courses as required by the States of Guernsey.
Work collaboratively with all members of the multi-disciplinary team.
Chair team briefing meetings.
Promote local resolution of any conflict or differences of opinion and involve appropriate parties for mediation, negotiation or discussion as appropriate.
Provide commitment and leadership in relation to equality and appropriate senior level management of diversity issues, particularly as they apply to training, people management and service delivery.
Strategy Business Planning:
Work with medical managers and general managers to take forward the business planning and objective-setting process for Public Health and Health & Social Care and represent the sexual health and HIV needs at appropriate clinical networks/other external clinical meetings.
Attend and contribute to regular Departmental meetings.
Performance Management:
Work with colleagues to ensure high performance in the following areas (where
applicable):
- Clinical efficiency e.g., relating to caseload and case mix of patients seen in clinic.
- Quality of outcomes e.g., partner notification rates, HIV testing rates.
- Financial management e.g., identification, implementation and achievement of cost improvement programmes and participating in efforts to ensure services are provided cost effectively e.g., monitoring and managing the drug budget to target, ensuring accuracy of clinical data for the team.
- Review clinical outcomes in designated areas using external benchmarking data where appropriate, to identify and advise variances to the Director of Public Health and participate in clinical audit, incident reporting and analysis and to ensure resulting actions are implemented.
Work closely with the Director of Public Health Medical Officer of Health (DPH/MOH) to participate in ensuring BASHH, BHIVA, FSRH and NICE and other national guidance requirements are reviewed and implemented and monitored, as far as possible in an isolated jurisdiction.
Ensure clinical guidelines and protocols are adhered to by nursing staff and any other staff for whom the post has responsibility and that these are updated on a regular basis.
Keep fully informed about best practice in specialty areas and ensure the implications for practice changes are discussed with the Medical Director.
Role model good practice for infection control to all members of the multidisciplinary team and adhere to the Infection Control measures.
Report all clinical and non-clinical accidents or incidents promptly, and to co-operate accordingly with any investigations undertaken.
Continuing Professional Development:
The successful candidate will be expected to comply with the relevant Royal College CPD requirements. 1 SPA is included in the indicative job plan to allow for activities related to revalidation, including CPD, mandatory training, appraisal, quality improvement activities, etc.
Appraisals:
Revalidation takes place in the normal way with doctors providing evidence in the GMC domains over the appraisal cycle. Medical appraisal for revalidation takes place annually in the normal way. Currently there is a mix of on-island and off-island appraisals during a five year cycle.
