Job summary

The States of Guernsey are looking for a Reablement Support Worker (Band 3) to join the team at Adult Community Services.The aim of the Reablement service is to assist individuals who are returning home from hospital or to work with them to prevent admission to hospital. The team assist by helping people to regain their independence through Reablement support. Reablement is short term support which gives the opportunity, motivation, and confidence to relearn or regain some of the skills required for daily living, which may have been lost because of illness, accident or disability.

Main duties of the job

The purpose of the job is to:

Provide a short, intensive reablement service for people with disabilities and thosewho are frail and recovering from illness or injury, focusing on helping people torestore their optimum level of independence.

Motivate individuals and support them to build confidence through identifying theirgoals and providing positive feedback about their progress and achievements.

Work with the Occupational Therapist, Physiotherapist, nurse, and social worker, tohelp deliver an agreed reablement plan, to help Service Users identify and achievetheir goals relating to a range of social care tasks such as washing and dressing,showering, meal preparation and helping people engage in community activities.

Provide a homecare service for people at the end of their reablement, pendinghandover to an ongoing care provider.

Provide support to adults to develop and maintain activities of daily living.

Job responsibilities

To support service users in a structured and consistent way to undertake a range ofsocial care tasks such as washing, dressing/undressing, showering, and bathing,meal preparation, domestic activities, engaging in community activities, working toa graded programme to help optimise their level of ability and independence.

To deliver agreed individual reablement programmes (with guidance and instructionfrom the therapists and managers) which includes tasks such as identifying suitableassistive technology equipment and demonstrating the safe use of such equipment,exercise programmes, advising on strategies for managing fatigue and pain,preventing falls, using adaptive techniques e.g. one handed or seated activities. Inthe delivery of all support, ongoing risk assessment to ensure service user andworker safety is prioritised.

To use supportive and motivational approaches to help service users identify andachieve their goals and build confidence, ensuring the delivery of a quality service.

To undertake a range of delegated health care tasks as required, includingprompting or the administration of prescribed medication support, the applicationof prescribed creams, eye and ear drops, catheter care, prompting exercises asprescribed by the physiotherapist. To complete and maintain accurate records asis necessary in respect of these tasks.

To work with health colleagues to deliver an integrated service where this hasbeen identified as beneficial to the person.

To provide written observations of the persons circumstances, whetherimprovement or deterioration, to help with the progression and review of thereablement programme.

To respond to any emergency situations, by taking the appropriate action e.g. Contactinga doctor or the ambulance service etc.

To liaise closely with Reablement Team registered staff and Reablement TeamManager, promptly informing them of any circumstance that requires animmediate response.

To attend Supervision, Annual Development Reviews, Team Meetings and Trainingas instructed to maintain high standards and compliance with regulations. This willinclude e-learning courses, attending venues outside of the service, attendance onnon-working days and at times undertaking training in personal time.

To maintain a high standard of administrative procedures by adhering to systems,completing accurate and comprehensive recording / documentation on TRAK (orother electronic record system) that evidence delivery of a reablement care andsupport service. This includes reablement support records, timesheets, mileageforms and annual leave requests.

To identify, record and report all incidents / accidents, potential safeguarding (safeguarding incidents to be reported in via registered staff) and any other highrisk situations where a Service Users safety may be at risk.

To respect the confidential nature of the work and protect personalinformation in accordance with General Data Protection Regulation.

To adhere to safe moving and handling procedures in line with individualsupport plans following training.

To contribute to the development of person-centered planning, and work in aperson-centered way, ensuring each individuals plan has meaningful andachievable goals with positive outcomes that meet aspirations as well as needs.

Where needed, support individuals to get in or out of bed, with dressing, undressing,

going to the toilet, eating, drinking, showering, bathing, hair washing in line withpersonal support plans.

Ensure all personal care tasks are carried out with respect for privacy and dignityand in line with needs, wishes and preferences of the individual as defined in thesupport plan.

To ensure that services take into consideration individuals ethnic, religious, culturaland communication needs when providing reablement support, and to practice inan anti-discriminatory way.

Encourage the involvement of people being supported in the day to dayrunning of their home, and to increase independent living skills.

Develop positive working relationships with family and carers.

To ensure the service is delivered in accordance with the policies, objectives andstandards of the States of Guernsey, The Office for the Committee for Health andSocial Care.

To positively promote reablement and contribute towards servicedevelopment to meet individual and service outcomes.
