Job summary

The States of Guernsey is seeking a Health Representative to join the Multi Agency Support Hub.

The postholder should be a SCPHN (school nurse or health visitor) with an interest in safeguarding to join the Multi-Agency Support Hub (MASH) as its health representative. The role is instrumental in sharing and analysing health information in order to make decisions within the MASH team and then feeding back to the health agencies involved with that child or young person and their family.

Day to day you will work as part of the multi-agency team, collating and sharing health information for the referrals received regarding children from across the Bailiwick. After collecting the health information form various sources you will analyse, share and discuss in the multi-agency team and then feedback the outcome from MASH in order to safeguard and promote the welfare of the child.

Management and safeguarding supervision is provided as well as ongoing access to training and development with an expectation this will be accessed regularly.

Main duties of the job

The postholder will work as part of a dynamic multi-agency team, assessing referrals regarding children aged 0-18 years that are received by MASH. This will include searching for, collating and analysing health information from a range of health services. With due regard for patient confidentiality, the postholder will need to interpret and share the information relevant to safeguard and promote the welfare of the child.

The postholder will be expected to feedback any information from MASH to relevant health professionals and to engage in health based supervision.

MAIN DUTIES AND RESPONSIBILITIES

The postholder will be an integral part of the MASH Team working alongside multi-agency colleagues in the MASH Team and having direct contact with children and their families where there is a health related aspect.

The postholder will collate and share relevant health information, explain its significance and participate in the discussions around risk and the promotion of the welfare of the child.

Facilitate the attendance of the most appropriate health professionals at pre-birth planning meetings, discharge planning or strategy meetings as required.

Participate in the MASH multi-agency risk assessment to inform the case management. Advising where necessary and challenging decisions when appropriate

Record MASH outcomes in RIO and disseminate decisions and appropriate information to relevant health professionals, including as necessary named nurse/doctor according to NMC guidelines..

Explain clearly and with sound rationale highly complex safeguarding issues to a range of professionals, particularly within Health, and health issues to other agencies as needed.

Immediately report to the Named Nurse for Safeguarding when issues require escalation, where there is a group of children involved (contextual safeguarding) or if the case is likely to become subject to public interest.

Assist in the evaluation and analysis of the health contribution to the MASH process.

Identify service gaps and issues relating to effective clinical pathways and/or cross-boundary arrangements. Escalate evidence of ineffective safeguarding arrangements within or outside of MASH to the appropriate individuals.

Develop and support a member of the Health Visiting Team to be a deputy to provide cover for annual leave, training and sickness as required and to be able to effectively record across both the health and social work IT systems to document relevant information.

Participate in health based safeguarding and managerial supervision as well as case management by MASH Deputy Manager/Manager

Participate in relevant working groups , services and initiatives to provide information, analytical advice and expertise.

The post holder may be expected to carry out other duties as may reasonably be required by their line manager.

THE ROLE

Communication:

Communicate decisions made MASH back to health agencies and professionals both verbally and in writing as appropriate.

Make links with GP surgeries to enable the transfer of relevant information between them and the MASH team, particularly before and after Strategy meetings.

To be able to challenge and discuss contentious issues with professionals and families to ensure the best outcome for children and young people referred to MASH, escalating to Named Nurse where necessary.

To communicate with service users of varying educational abilities, social circumstances and different cultural backgrounds, in order to maximise the wellbeing and minimise risk to the child and family.

Develop constructive relationships with a broad range of Health and Care professionals as well as third party agencies ensuring that this role is promoted and in order to achieve good outcomes for children and families.

To maintain accurate contemporaneous records on RiO and MOSAIC in line with NMC guidance and local policies.

To provide verbal and written reports, including outcomes of MASH actions allocated to Health.

Be aware of the MASH process and competently refer to the MASH threshold guidance and continuum of need whilst collating health information and providing an analysis of this for referrals

Clinical

Offer health advice regarding families referred to MASH, including the identification of normal/abnormal development, recognition of post-natal depression indicators.

Offer health support on behalf of MASH to families where this is appropriate.

Under the guidance of MASH Manager/Deputy, make contact with referrer and mother-to-be when ante-natal referrals are made to collate information in order to inform the MASH process. Be able to signpost families to different services available both antenatally and following the birth of the baby.

To prescribe using the Nurse Formulary to ensure prescribing is undertaken in a safe cost effective manner.

MANAGEMENT/LEADERSHIP

Contribute to the continuing development of MASH process and services to children and families, particularly those relating to health and support in disseminating any new initiatives to the wider multi-agency teams.

Develop pathways for information sharing to ensure that all information is shared with appropriate health professionals at appropriate times.

Liaise with Team Manager, Named Nurse and other members of the Community Childrens Teams regarding the role as Health Representative at MASH and any proposed changes to the MASH process.

To provide specialist health related advice to other professionals in relation to the safeguarding of children.

Participate in regular supervision with Named/Designated Nurse for Safeguarding, Team Manager and MASH Manager/Deputy Manager.

Be aware of Incident Reporting Policy and be able to report any relevant concerns in relation to safety and well being of staff and children/families.

To be aware of and act in accordance with Health & Social Care (HSC) operational policies and procedures.

Develop and support a deputy to be able to offer cover for annual leave, training and sickness.

Participate in yearly health management appraisals as per HSC policy.

TRAINING

To participate in the Induction of new staff.

To undertake statutory training on an annual basis.

Keep up to date with new research regarding Health Visiting/School Nursing and safeguarding practices, be aware of outcomes and lessons from serious case reviews.

EDUCATION AND EXPERIENCE

Qualifications required:
