Job summary
The States of Guernsey is seeking a Band 6 Senior Staff Nurse for Adult Community Servicey to join their team at Adult Community Services.The roll will entail being respoonsible for overseeing appropriate standards of assessment, planning, implementation and evaluation programmes of care delivered to a group of patients/clients within the community setting.
Main duties of the job
Is responsible for overseeing appropriate standards of assessment,
planning, implementation and evaluation programmes of care delivered to a group of patients/clients within the community setting
Act as a leader, advisor and role model to less experienced nurses,
ensuring appropriate care is implemented and maintained.
Be involved in implementing and monitoring quality assurance
initiatives. This includes devising action plans and measuring outcomes.
Be involved in the development of, and participate in, teaching
programmes for qualified and unqualified staff and students.
Act as an assessor to students, mentor to newly qualified staff and
clinical supervisor.
Participate in the delivery of professional development programmes to meet the needs of registered and unqualified nursing staff.
Take charge of the twilight /night service on a regular basis acting as
the nominated deputy to the person with twilight/nights accountability.
Covering twilight and night duty act as duty nurse for adult community services
Assist the band 7 to undertake individual performance review of nursing staff at Band 5 and below.
Work toward attaining specific objectives that are agreed with and
reviewed by the team leader on a regular basis.

CLINICAL:
Organise the effective acceptance of referrals to the team using the TRAK system
In conjunction with the District Nurse/Team Leader, supervise and advise nursing staff within the department on the management, organisation and delivery of nursing care.
In conjunction with the District Nurse/Team Leader ensure that agreed policies, protocols and procedures are adhered to in the delivery of patients/clients care.
Ensure colleagues are maintaining standards of care by carrying out regular audit/evaluation of clinical standards and agreeing action plans. Take appropriate action where performance does not meet required standards.
Take reasonable measures to ensure that own and others nursing care is based on current sound evidence based research.
Maintain accurate records of care in accordance with the NMC Standards, clinical standards and Health and Social Care policies using paper records and TRAK recording systems
Assess individual patient/client handling, transferring and lifting needs, documenting how they will be met safely and effectively on the care plan, and ensure these are evaluated and reviewed as the patients condition changes.
Communicate information to ensure the management of patient care is appropriate and adequately understood by those who are expected to deliver it.
Take all reasonable steps to safeguard the welfare and safety of patients, staff and oneself in accordance with Health and Safety at Work (General) Guernsey 1987.
Plan for the effective safe discharge/transfers of patients and liaise with other appropriate health professionals and agencies
MANAGERIAL:
Act as leader to a team of nurses giving them advice and support as necessary.
Be familiar with the role and responsibilities of the District Nurse/ team leader and be able to manage delegated responsibilities in his/her absence.
Support the District Nurse/ team leader in implementing and managing change or new initiatives.
Assist the District Nurse to undertake individual performance review of nursing staff at Band 5 and below.
Report and record all accidents according to policy taking appropriate action.
Report at time of occurrence any incidents or complaints and complete accurate records.
Be conversant with the fire, MIP, and other emergency procedures and be able to lead to take the appropriate action.
Be knowledgeable and adhere to agreed procedures to maintain community equipment, report any delays in action or suggest need for replacement to District Nurse
EDUCATIONAL:
In conjunction with the District nurse and practice development, develop training programmes to enable students and learners to achieve objectives.
Act as assessor to students and learners, and give support to other assessors within the area.
Act as mentor and supervisor to newly qualified staff allocated to work on the community and assist them to identify their learning needs
In conjunction with the practitioner development facilitate programmes to meet the learning needs of new staff members and assess their competence levels.
Participate in teaching sessions within the clinical area and, if approved by the Service Manager, at the Institute of Health Studies.
Assist the District Nurse/Team Leader in identifying the educational and developmental needs of all staff.
Assist the Team Leader and Service Manager to implement clinical supervision on the twilight/night service
COMMUNICATION:
Maintain and evaluate effective communication links with all disciplines, teams and departments.
Organise and chair meetings as agreed with the Team Leader
Attend and participate in departmental and team meetings.
PROFESSIONAL DEVELOPMENT:
Identify own educational and professional development needs by a means of reflective practice and formal appraisal
Participate as required in the recruitment and selection procedures of new staff to the department including department induction
Assist with the co-ordination of duty rosters for all staff.
Adhere to NMC Code of Professional Conduct and adhere to NMC standards. Be aware of advisory papers published by NMC that are appropriate to area of practice.
Ensure NMC Registration is maintained and revalidation is under taken 3 yearly.
