Knowledge, Training & Experience Required

Professional knowledge acquired through a masters degree in pharmacy, with a postgraduate clinical diploma qualification or equivalent.

Registered as a Practising Pharmacist with the UK General Pharmaceutical Council (GPhC) and adherence to the standards of the Code of Ethics for Pharmacists.

A minimum of 3 years experience of primary care (CCG or GP surgery) and hospital pharmacy.

Non-Medical Prescriber (training will be provided if not a NMP)

Knowledge of the principles of health economics and clinical epidemiology.

Medicine Information skills (i.e. analysing and abstracting complex clinical and scientific information from primary, secondary and tertiary sources).

Audit experience.

Evidence of relevant continuing professional development.

Experience in the use of EPACT and Scriptswitch desirable.

Main Duties and Responsibilities

The following is an outline of the tasks, responsibilities and outcomes required of the role. The post holder will carry out any other duties as may reasonably be required by their line manager. The job description and person specification may be reviewed on an ongoing basis in accordance with the changing needs of the organisation.

The post holder will work autonomously under the direction of the Associate Director for Pharmacy and Chief Pharmacist, along side the PSU as required

The post holder will design, develop and implement appropriate work plans/directives across primary and secondary care including doctors, pharmacists, nurses, acre homes and practice staff.

The post holder will work autonomously to set, prioritise and manage own workload across care settings including care homes and GP practices and secondary care and will be responsible for meeting the objectives set.

The post holder will participate on the development and co-ordination of training programmes across the primary/secondary care interface including care homes and community pharmacies.

The post holder will cover clinical pharmacy duties within the hospital including ward cover and dispensary.

Activities

To be the lead on deprescribing and reducing polypharmacy by designing, agreeing and implementing appropriate evidence-based strategies.

To improve the prescribing across the interface, identifying areas for reducing errors , cost savings and improvement in quality, using national and local clinical guidance e.g. NICE and their own experience.

To design and carry out audit, creating reports, implementing change and providing training to relevant healthcare staff based on the findings of those audits.

To be the identified interface between Pharmacy and the wider healthcare staff, staff in other settings and the third sector.

To monitor best practice guidance, and to work to amend/implement local medicine-related legislation or policies as required.

To design, implement and monitor cross-sector working between primary and secondary care, including shared care guidelines and protocols using evidence and personal experience to overcome barriers.

Clinical

Support safe effective and economic use of medicines:

To implement agreed prescribing changes to reduce errors and to ensure high quality and cost effective prescribing.

To provide expert advice on a strategic level to HSC on prescribing issues related to deprescribing and polypharmacy.

To develop, implement and report back on prescribing initiatives which will:

Reduce wastage and hence spend on prescribed drugs.

Improve prescribing in line with national guidance.

Reduce medication-related errors.

Provide clinical pharmacy services to the hospital as required.

Leadership

To collaborate on transformation of and innovation in service delivery.

To attend and advise relevant HSC committees, recommend mechanisms for inter-nal and external audits as required providing advice to medical staff on medicines usage and expenditure.

To be a member of the HSC Committees as required

Line management of pharmacists/technicians as required

Training, education and research

To actively participate in Continuing Professional Development.

To attend training courses and conferences as appropriate and provide feedback to colleagues.

To develop and deliver training to practice teams, care home staff and other multidisciplinary staff as appropriate on the safe and efficient management of medicines. (e.g. repeat prescribing systems).

To promote positive working between care homes, GP practices, community pharmacists and other health professionals in relation to medicines management.

To Partake in on call and weekend working services for HSC

Qualifications

Essential

• 1.Masters Degree in pharmacy or equivalent.
• 2.General Pharmaceutical Council Registration.
• 3.Hold or be working towards a post-graduate diploma/masters degree or equivalent relevant to pharmacy practice.
