Job summary

We are looking to appoint a Clinical Nurse Specialist who will provide pain management to patients with acute, sub-acute and chronic pain conditions within the Bailiwick of Guernsey. The CNS Pain Management will work with all members of the multi-disciplinary team to ensure the continuity of pain relief is provided that is holistic, effective and individualised to meet the patients need. The post is a band 7 role with the possibility of progressing to a band 8A if the right competencies are achieved and if funding can be secured.

Main duties of the job

Will provide pain management to patients with acute, sub-acute and chronic pain conditions within the Bailiwick of Guernsey. The CNS Pain Management will work with all members of the multi-disciplinary team to ensure the continuity of pain relief is provided that is holistic, effective and individualised to meet the patients need. For example, this would include daily inpatient ward round, consultants ward round, qutenza clinic. Outpatient titration, weaning medication clinics, transition clinic and Alderney clinics with adhoc patient reviews post discharge and to participate in weekly integrated team meetings.

The primary responsibility will be to work with the nurse consultant to ensure that patients with both acute and chronic /complex pain needs receive the appropriate evidence-based interventions and education they require. To ensure that the service evolves in line with local and national guidance. The Service is being moulded inline with the guidelines for provision of anaesthesia services for integrated inpatient/outpatient pain management services 2021.

Lead in the provision of specialist knowledge and education in the field of acute/chronic, persistent pain management.

Be able to deputise for the Consultant Nurse in Pain Management and to participate in service development under the supervision of the Consultant nurse in pain management.

Job responsibilities

To work independently / interdependently within the scope of professional practice in the specialty of acute and chronic pain/persistent pain in both inpatient and out patent settings. This includes guiding the practice of pain management through education, support and supervision of staff.

To demonstrate excellent communication and interpersonal skills, participating in strategies and initiating change where appropriate to improve the quality of pain management services.

To clinically assess, plan, implement and evaluate programmes of care, providing specialist advice and actively seeking evidence-based practice which will improve the quality of care and meet the needs of a group of patients. A holistic bio-psychosocial approach to caring should be planned that will include the appropriate level of physiological, psychological, spiritual, socio-economic and cultural aspects which influence the care of patients within this speciality

On-going follow-up and supporting of patients with complex pain conditions. Including the management of epidurals, PCAs, ketamine and lignocaine infusions titration, rotation and weaning of analgesics, in collaboration with anaesthetic and pain colleagues as required.

Facilitate short and long-term follow up of patients through provision of nurse-led pain clinics, telephone triage.

Review each patients response to treatment by regular contact, monitoring effect and ensuring early detection, and management of side effects from proposed medication changes.

Manage episodes of care as own/joint shared caseload.

Ensure that accurate and comprehensive records are kept and confidentiality is maintained. To keep statistics and participate in audits.

Lead the development of professional nursing practice policy/guidelines within the speciality, demonstrating commitment to share expertise and disseminate information.

Maintain and update own clinical expertise and contribute as role model to ensure the delivery of nursing care is at highest possible level to meet patient care.

Assess new patients and facilitate them in setting personal goals and reviewing medications and pacing strategies.

Promote patient education and understanding of pain, rehabilitation and self management strategies, on an individual basis and as part of the pain management or other treatment programmes. To develop educational materials where required and to be able to audit outcomes against established KPIs.

Educate and train staff on current evidenced based acute and chronic/persistent pain management practice.

To receive appropriate direct referrals from all health care doctors/physios nursing staff.

Where appropriate prescribe, if you have undergone non-medical prescribing training.

To assess, monitor and challenge risk in own and others practice.

To work closely with other members of the wider multidisciplinary team and participate in appropriate team meetings.

Be aware of the importance of professional communication and personnel factors, which can positively and adversely affect the delivery of the pain management service.

To be able to rotate from the inpatient pain service to the community pain service if required by the Nurse Consultant.

To give clear and comprehensive handover of information relevant to medical staff regarding patients in the care of pain services, to promote continuity in the delivery of the service.
