Job summary

An exciting opportunity is available in Guernsey for an experienced Critical Care nurse to train as an Advanced Critical Care Practitioner.

We are currently an 8 bedded unit providing services for all high dependency and critically ill patients within the Bailiwick. A new 12 bedded Critical Care unit is currently being built and expected to open early next year and we are therefore looking to expand our current ACCP team consisting of two qualified practitioners and two trainees.

The successful applicant will be appointed to a 2-year band 7 training post, during which they will undertake the MSc Advanced Critical Care Practitioner at the University of Southampton and undertake the FICM competencies under the supervision of a consultant Intensivist in Guernsey. Following successful completion of the academic and clinical components of the course they will be employed as qualified ACCP at band 7, progressing to an 8a upon completion of the dissertation module and obtaining a full Masters degree. It is important to note that whilst we follow the FICM trainee ACCP curriculum, applicants will not automatically be eligible for FICM accreditation as our unit is outside of the NHS and not a centre approved for Intensive Care Medicine Training. However this may be possible after undertaking a further period of study/clinical attachment to be confirmed by FICM.

Main duties of the job

In this advanced nursing role, the post holder will develop expert clinical management andcare to patients requiring levels 1-3 care in Critical Care and for acutely unwell patients onthe wards, in a role traditionally performed by medical staff.

The post holder will work clinically within critical care, in a protected supernumerarytraining capacity for 2 years, alongside completing a bespoke Post Graduate Diploma inAdvanced Critical Care Practice and including non-medical prescribing.

The post holder will receive training and supervision form a ConsultantIntensivist/Anaesthetist in completing clinical competencies and carrying out practicalassessments as defined by the Faculty of Intensive Care Medicine, within the workplace.

Theoretical work will be assessed by the Higher Education provider. It should be noted thatthis training post will not entitle the post holder to become an Associate Member of FICM.

Upon successful completion of the two-year training period the post holder will beappointed to a Band 7 qualified Advanced Critical Care Practitioner position and uponachieving a full Masters Degree upgraded to a band 8a.

Demonstrates comprehensive knowledge across a range of subject areas relevant to the field of critical care.

Critically analyses, evaluates and synthesises different sources of information for the purpose of assessing and managing the care of a critically ill patient.

Applies the principles of diagnosis and clinical reasoning that underlie clinical judgement and decision making.

Communicates complex information to patients/carers during the consent process prior to commencement of treatment plans and invasive procedures including explaining treatment options.

Proactively supports patients and carers in coming to terms with their illness/condition.

Ensures effective communication is maintained between members of the multidisciplinary team to ensure appropriate individuals are informed of changes to the patients conditions or treatment plan.

Provides empathy and reassurance through understanding the quality of life issues associated with their illness/condition.

Supports the Consultant Intensivist and other medical staff, in providing patients with treatment plans on critical care.

Exchanges information with patients and staff regarding appointments, admissions, discharges and follow up care in verbal and written form.

Maintains accurate and up to date paper and electronic patient records ensuring security of such information.

FREEDOM TO ACT

The Trainee Advanced Critical Care Practitioner will have a high degree of autonomy and authority to make clinical decisions, supported by an appropriate medical professional within the scope of his/her professional practice. During training they will work under direct/indirect supervision at all times.

The ACCP will plan their workload using organisational and National guidance, policies and professional code of conduct in accordance to the needs of the patient group.

When qualified as a non-medical independent prescriber, may prescribe medications within their sphere of competence and in accordance with local policy.

Uses own initiative, acts independently and take appropriate actions.

REVALIDATION

In accordance with States of Guernsey legislation and the HSSD procedure the Registration of Health and Social Care Professionals (COR001), all nurses and midwives who are HSSD employees, contractors, agency workers or in any other capacity are practising as a nurse or midwife are required to have effective registration with the (UK) Nursing & Midwifery Council (NMC).

In order to maintain effective registration with the NMC you must adhere to the Council's requirements for Revalidation.

You must proactively manage all aspects of your Revalidation requirements including keeping an up to date portfolio and proactively seeking out opportunities for reflection, continuing education and professional growth. You must be proactive and cooperative in the appraisal and confirmer processes.

You must assist colleagues with relevant aspects of their Revalidation. This includes but is not limited to teaching, providing support and opportunities for Facilitated Reflective Practice, participating in professional discussions and providing constructive feedback.
