Job summary
The States of Guernsey are looking to recruit a Lead Cancer Services and Aseptics Pharmacist (Band 8b) to join the Pharmacy Team at the Princess Elizabeth Hospital.
The Lead Cancer Services Pharmacist, as the expert in oncology, will provide a dynamic link between clinical practice and service development within the Princess Elizabeth Hospital. They will ensure patients receive the best experience and outcomes from their medication.
Main duties of the job

To provide a specialist oncology pharmacy service optimising patient outcomes and improving the patient experience, working within patient care pathways
To contribute to improving health outcomes for patients using a holistic approach to are, ensuring the safe and effective use of medicine through provision of medicines-related aspects of patient care
To promote improvements in quality, safety and cost effectiveness of medicines use and supporting holistic management of clinical conditions, influencing the wider community of practice
To act as a mentor and role model as a clinical pharmacist, manager, and leader
To lead and deliver education and training in oncology for pharmacy and other healthcare professionals across the health economy
To be the Accountable Pharmacist within the Aseptics unit
To ensure all QC and QA requirements are met for EL 97 52 inspections
To promote the safe, rational and cost-effective use of medicines by working closely with nursing, medical staff and health care managers.
To support patients to achieve their health goals in relation to medicines by providing person centred care

• To Participate in clinician ward rounds and multidisciplinary team meetings within oncology.

• Assessing the appropriateness and safety of prescriptions in terms of dosing, biochemistry, disease management and allergy and recommending changes to treatment where appropriate to improve effectiveness of drug therapy and patient outcomes.

• As an independent prescriber, provide dose adjustment, advice and prescriptions for SACT medication with a narrow therapeutic range requiring therapeutic effect monitoring, including recording relevant biochemistry results in relevant patient medical records. Recording action plans and adjusting doses to account for renal or liver dysfunction or other toxicities.
• To ensure all required quality assurance and quality control activities are completed to a satisfactory standard
• To review all deviations and CAPA activities within the aseptics unit, alongside the lead aseptics pharmacy technician.
• To monitor levels of pharmaceutical waste, both from expired medication and from medication not passing quality control
SERVICE DEVELOPMENT
• To contribute to the production and implementation of business plans, and to contribute to business case development within the Pharmacy and oncology departments, liaising with the pharmacy senior staff to ensure any impact on the clinical and operational aspects of pharmacy services are fully identified and resourced
• To identify new prescribing practices and ensure and monitor the safe, efficient and economic use of medicines within oncology, in accordance with national and local guidelines, working with appropriate colleagues.
• Contribute to strategic and contingency planning for the service, which may have an impact on other areas.
• To keep abreast of and be at the forefront of developments in oncology pharmacy practice and relevant general practice
