We are looking for an organised, motivated and supportive Band 5 Primary Healthcare Practice Nurse to join our phenomenal team here at Catterick Medical Group, based at Vimy Barracks, Catterick Garrison.

This role offers 37.5 hours per week and requests to work flexibly will be considered, taking into account business needs.

Main duties of the job

As the chosen candidate, you will work as part of a multidisciplinary primary healthcare team delivering primary healthcare nursing including elements of occupational health. You will be working with the Senior Practice Nurse or Senior Medical Officer within the medical facility.

As the post holder, you will be providing high-quality person-centred care, always considering people’s safety, privacy and dignity using advanced clinical knowledge that is evidence based. You will also be monitoring and improving standards through supervision, evidence-based practice, clinical audit, research, and education.

Job description

Job responsibilities

Duties and responsibilities for the role include but are not limited to:

  • Support the care of patients with chronic illnesses e.g. Asthma, Diabetes, Chronic Heart Disease (CHD) and hypertension ensuring continued maintenance of the Chronic Disease Register.
  • Assist with the development of plans to meet the health needs of patients including diagnostic tests; treatment plans, health promotion and prevention.

  • Plan, manage episodes of care, working in partnership with others and delegating and referring as appropriate to optimise health outcomes and resources use, as well as providing direct support to patients and clients.

  • Responsible for the generation and collation of reports on own practice and that of team on service delivery and local and national targets, and monthly reporting to Healthcare Governance and other relevant Meetings.

  • Strive constantly to improve practice and health outcomes so that they are consistent with or better than national and international standards through initiating, facilitating and leading change at individual team, organisational and system levels.

  • Contribute to and at times lead the development of multidisciplinary research and clinical audit.
