Job responsibilities

Responsibilities of the role:

Leading with the care of patients with chronic illnesses e.g., Asthma, Family Planning, Diabetes, Chronic Heart Disease (CHD) and hypertension ensuring continued maintenance of the Chronic Disease Register.

Managing the development of plans to meet the health needs of patients including diagnostic tests, treatment plans, health promotion and prevention.

Assisting the practice team with maintaining an effective recall system for Immunisation/Chronic Disease/Cytology.

Providing specialist advice and support to patients, their families and other healthcare professionals following assessment, diagnosis and through treatment.

Initiating pharmacological intervention, using non-medical prescribing in accordance with DPHC policies and current legislative frameworks.

Qualifications

Essential

• Registered Nurse on Part 1 of the Nursing and Midwifery Council register
• Current registration with the Nursing and Midwifery Council
• Advanced Practice Nurse Qualification / post graduate diploma in Advance Practice or successful accreditation with the Royal Collage of Nursing (RCN) or actively working towards this and can demonstrate equivalent level of competence and experience in clinical assessment and diagnosis)

Experience

Essential

• Relevant teaching qualification or can demonstrate equivalent level of competency
• Learning and Assessing in Practice Qualification or equivalent practice assessors training
• In date for Basic Life Support (BLS) Anaphylaxis, Automated External Defibrillation (AED) Infection Prevention and Control (IPC) Safeguarding Level 3 (Children and Adult depending on population at risk) or willingness to undertake training
