Job summary

We are looking for a Community Staff Nurse in the Health Visiting Team to join our award-winning service in the Wells, Street and Glastonbury team. Using your lived experience, you will be able to get involved in service development and share ideas and best practice.

  • The salary for this post is in line with Agenda for Change Band 5, which has a current salary range of £28,407 - £34,581 per annum.
  • Part time: 22.5 per hours per week
  • Membership of the NHS Pension Scheme, Subject to meeting certain criteria, or Auto enrolment onto our generous Pension Scheme and optional pension enhancement through our Additional Voluntary Contribution scheme.

Main duties of the job

We are a team with a strong Public Health Focus committed to improving the health and well-being of our children and families in Somerset using good leadership skills and a sound knowledge base in safeguarding and child protection processes.

You must be an NMC registered Nurse or Midwife and an interest of holistically addressing the health needs of children aged 0-5 years and their families.

Applicants must have a full valid driving licence and have access to a car for work. If necessary, adjustments can be considered in line with the Equality Act 2010.

Job responsibilities

1. To work to the NMC code of professional conduct, understanding the importance of professional accountability, responsibility and competence.

2. To contribute to the implementation of care in line with agreed standards and through evidence-based practice and report to the relevant line manager all instances where such standards cannot be met.

3. To deliver care in accordance with Organisational Policies, procedures and guidelines as delegated by an allocated Health Visitor/School Nurse.

4. To contribute to the identification of children in need and deliver care in accordance with the Organisations Safeguarding policies and procedures

5. Work as a member of the Health Visiting service within a designated locality supporting the Health Visiting Team to deliver on public health priorities in Somerset to children and families in line current national Public Health Agendas, working to National and Local objectives as defined by Service Specifications under the supervision and guidance of a Health Visitor.

6. Work with vulnerable children and families in line with national and local policies relating to safeguarding children and to provide specialist support and advice to children, families and schools regarding the health and wellbeing needs of children.

7. Work in partnership with other agencies, including Midwifery, GPs, Paediatricians, NHS Providers, Schools, Early Years Settings and Childrens Social Care. Support the delivery of programmes of intervention for individual children and families.

8. Deliver National and local programmes on an individual, group and community level. Supervise and guide the Health Visitor team assistant practitioners and administrator.

9. Be aware that this is a development post and proactively seek out of relevant learning opportunities in order to progress learning and competence.

Key Responsibilities

Under supervision and guidance of the Public Health Nurses:

1. Participate in the delivery of the National Healthy Child Programme 0-19 years, including developmental checks, child health clinics, group work and supporting baby clinics.

2. Identify need through Family Health Needs Assessment and Child Health Needs Assessment and provide personalised advice, support and interventions to parents, carers and schools regarding health and social issues. Complete contemporaneous electronic record keeping including report writing and client records in line with the organisations policies and NMC code of Practice.

3. Participate in the promotion and delivery of young peoples clinics in partnership with the educational and community establishments to promote general health and wellbeing of children and young people.

4. Work collaboratively as a member of a multi-disciplinary team with other agencies to maximise the Public Health Nursings service contribution to health improvements in the local community.

5. Facilitate and deliver targeted groups in a variety of settings.

6. Contribute to the delivery of health and social learning programmes for young people with identified needs.

7. Contribute to the delivery of care programmes for vulnerable children in conjunction with the Public Health Nursing Team, families, school and other professionals.

8. Contribute to Health Visitor / School Nurse Liaison regarding children who may have additional needs relating to transition between pre-school and school and support the transition.

9. Understand, uphold and promote the aims of the councils equality, diversity and inclusion policies; including health, safety and wellbeing of self and others and organisational values in everything you do. Equality and Diversity practice covers both interaction with staff, service users and communities and includes challenging discrimination and promoting equality of opportunity for all. 10. Complete a period of induction to achieve the competences required to undertake the role.
