Main duties of the job
As a remote staff nurse working on our centralised duty system, you will have the opportunity towork with a team of experienced health professionalsto provide high-quality care and support to children and families.
Your day-to-day work will involve:
- Triaging, reviewing and assessing patient needs.
- Developing care plans.
- Delivering nursing interventions to promote health and wellbeing.
- Responding to phone calls from professionals as well as parents and carers.
We offer ongoing support, training and guidance to help you be the best you can be. You will need to have:
- Anursing degreeand be registered with theNMC
- Excellent communication and interpersonal skills
- Excellent organizational and IT skills and able to work independently
- Experience working in the community or with children/families
You may also have a lapsed SCPHN qualification or experience working in a Public Health Nursing team.

- Support the Health
Visiting teams by working on centralised duty to deliver on public health
priorities in Somerset to children and families in line current national
Public Health Agendas, working to National and Local objectives as defined by
Service Specifications under the supervision and guidance of a Health Visitor
and duty manager.
- Answer the parent
health line, professional line and review and action emails alongside the
Health Visitor on duty. Work with vulnerable children and families in line
with national and local policies relating to safeguarding children and to
provide specialist support and advice to children, families and schools
regarding the health and wellbeing needs of children.
- Work in partnership
with other agencies, particularly the Local Authority, including children
social care and early years settings. Map
local Support the delivery of programmes of intervention for individual
children and families so you can signpost families where necessary.

the National Healthy Child Programme 0-5 years, including developmental
checks.
- Identify need through
Family Health Needs Assessment and provide personalised advice, support and
interventions to parents and carers regarding health and social issues. Complete contemporaneous electronic record
keeping including report writing and client records in line with the
organisations policies and NMC code of Practice.
- Providing
specialist advice and support on a range of issues such as child development,
nutrition and parenting skills.
- Work collaboratively
as a member of a multi-disciplinary team with other agencies to maximise the
Health Visiting service contribution to health improvements in the local
community.
- Identify and manage
safeguarding concerns and work in partnership with other agencies to provide
appropriate interventions.
- Liaise with School
Nursing teams regarding children who may have additional needs relating to
transition between pre-school and school and support the transition.
- Understand, uphold and promote the aims of the councils equality,
