Job summary

The primary function of the role is the day-to-day contract management of a portfolio of NHSE provider contracts for All Ages Continuing Care , including monitoring, performance management in accordance with any contractual or locally agreed timetable.

Main duties of the job

The post holder will report to the Contract Manager in a supporting role in monitoring, reporting and management of the key national and local quality and access targets

This will include building effective relationships with relevant professionals and key stakeholders e.g., Service users, relatives, assessors, Placement officer, local authority representatives and providers etc.
