Job summary

An exciting opportunity for a Clinical Lead Nurse has become available within the AACC team as the current post holder is retiring. The AACC team is an established service with a good track record for exceeding national KPI's, averaging 90% on the 28 day KPI. We are looking for a dynamic leader with sound clinical and leadership experience in All Age Continuing Healthcare.

Main duties of the job

  • Will be responsible for the direct delivery of the Continuing Healthcare service, developing and maintaining the highest levels of relationship management with customers and other stakeholders
  • To provide clinical leadership and operational management for the clinical pathway for AACC
  • To provide clinical expertise to the team and other stakeholders. For example, complex case management and Court of Protection applications
  • To ensure that allocated resources are effectively and efficiently managed.
  • To ensure the timely, effective, high-quality delivery of key service developments and KPI's

You will be joining the team at an important time and will have a big contribution to make towards improving assurance around the quality of the Clinical Service provided by the team. Working closely with administration/placement and financial colleagues and the broader team you will be able to effectively plan and manage the change required to improve the quality and consistency of the clinical service delivery.
