Job summary

Due to service expansion, we are looking for an individual with experience of working in the health or care sector to join the case management team within the continuing healthcare team, this is a permanent role for a 37.5hr per week contract, we would welcome applications from nursing associates.

The continuing healthcare team of over 40 staff delivers a service which undertakes the assessment and review of continuing healthcare. The case management team of over 10 people sits within the continuing healthcare team and support to clients who are assessed as eligible for continuing healthcare and our team supports people with the planning and implementation of plans to meet their health and wellbeing needs. The case manager has a key role in ensuring people, families and carers receive the advice, support, and guidance they need to achieve their goals and outcomes, offering choice and flexibility.

We offer hybrid working in this role meaning that the successful candidate can work from a mixture of locations including home, NECS office in Middlesbrough and customer locations.

Main duties of the job

This role will be responsible for:

Work in partnership with people and families/carers to build effective relationships, to gather their need and views and promote participation in decision making.

Undertake assessments

Plan, implement and review person centred support plans.

Supporting people through personal budgets to have choice and control over how their care is delivered and by who, with the option of a direct payment.

Support with the CHC fast track process, planning and commissioning care to support end of life care.

Implementation of the medication pathway, considering alternatives to traditional service delivery including the use of assistive technologies to support independence

Liaise, negotiate, influence, and collaborate with a variety of internal and external stakeholders.

Manage a workload independently and maintain accurate up to date records.

Pro-actively engage with colleagues and a range of organisations to identify, assess, plan for and support the needs of children, young people and families/carers.

  • Proven experience of managing financial aspects of programmes.
