Job summary

NHS North of England Care Systems Support (NECS) works across the UK to support health and social care customers in meeting their strategic and operational challenges, to improve outcomes and increase efficiency.

The Role

An exciting opportunity is now available in the Tees Valley Medicines Optimisation team in the role of practice based Medicines Optimisation Pharmacist.

The NECS Medicines Optimisation team consists of over 100 pharmacists, technicians and specialist support staff based throughout the North of England. Our friendly and welcoming Tees Valley team consists of over 30 staff providing specialist medicines optimisation support across Tees Valley, working with over 80 GP practices, care homes, local authorities and other health and social care providers.

Main duties of the job

This role will be responsible for:

· Working collaboratively with a wide range of primary care healthcare professionals and providers to improving patient care, quality and safety of prescribing

· Carrying out structured medication reviews, which can include:

o Care home residents

o Patients who have complex polypharmacy concerns and needs

o High dose opioids

· Carrying out clinical audit, and working with nurses, GPs, practice managers and other health care professionals in GP practices to ensure appropriate actions are taken .

· Facilitating the implementation of prescribing and medicines optimisation guidance within GP practices

· Using specialist tools to monitor and analyse prescribing patterns

· Building good working relationships across all pharmacy sectors – Community Pharmacy, Primary and Secondary Care

Responsible for providing support and assistance to GP practices to identifying areas of medicines optimisation that require improvement.

To support the implementation of change, including negotiation and delivery of training, in order to maximise cost-effective prescribing and to improve the quality of patient care.

To undertake patient medication reviews that will help patients get the best out of their medicines by identifying unmet pharmaceutical need, and assist in improving access and service efficiency whilst aiming to reduce waste medicines.

Responsible for encouraging the development of better medicines optimisation understanding throughout the health and social care teams and promoting good practice in line national and local guidance.
