Job summary
Due to growth, we are looking for an enthusiastic person to
apply for the role of LeDeR/Community Deprivation of Liberty Safeguards (COP
DoLS) Reviewer to join the All-Age Continuing Care & LeDeR team. The team
offers a range of clinical services to a wide array of clients across the
health sector, including ICBs, Acute Trusts, Health and Social Care partners,
individuals and their families. Part time and full-time posts available to be
agreed upon appointment.
The post is an amazing opportunity to join an ever-growing
Clinical Service, providing opportunities for growth, development and expansion
of knowledge and skills supporting your professional development. The post
holder will work in a team managed by the CHC team and we wish to develop the
opportunity for the post-holder to learn more about this area of work.
We want the LeDeR review team to be from various
professional backgrounds to offer the advantage of full MDT knowledge and
experience to every review. The postholder will need to be able to use this
knowledge to its best advantage to ensure LeDeR reviews are of the highest
quality and standard.
Hybrid working is offered meaning the successful candidate
can work from a mixture of locations including home, NECS offices and customer
locations.
Main duties of the job
Complete LeDeR reviews to the required quality standards through;
Assimilation of complex clinical information
Critical thinking and professional curiosity
Liaison with the wider health and social care team
Interviews with bereaved families/next of kin using advanced communication skills
Identification of issues and learning to inform local and national service improvement for people with Learning Disability and Autism
Working to internal and external deadlines using caseload management approach
Presentation of reviews at local quality panel meetings

Undertake LeDeR reviews which are comprehensive and complete in a timely manner, draw out learning and draft action plans for local implementation, identify reviews which require referral into statutory processes including safeguarding, and any other clinical decision.
Undertake complex reviews to ensure completion in a timely manner and agreement with ICB LACs.
Support the Band 6 in undertaking simple reviews to ensure completion in a timely manner and agreement with ICB LACs.
Accountable for peer quality assurance of the teams reviews according to end-to-end processes.
Ensure appropriate clinical governance of the process and its deliverables.
Support in the identification of cases that require multi-agency review.
Provide accurate and timely management of information relating to LeDeR reviews and ensure the team submit review progress updates into the information management system.
Through the Family Liaison Officer and project support staff, engage with families and advocacy services where appropriate to collect and collate the relevant clinical and social information in order to complete the review documentation to the required standard on the LeDeR system website.
Present the case at an appropriate panel.
The post holder will communicate effectively and sensitively with a wide range of care providers including the independent sector and voluntary sector where appropriate.
Analyse the findings of the initial review and identify recommendations and lessons learned for practice to influence commissioning and delivery of current and future service and service reform, within multi-agency partnerships and joint commissioning arrangements.
Collect and collate qualitative and quantitative data as required by the programme.
Challenge practice / service delivery and recognise the support needs of reviewers, Family Liaison Offices and Project Support Staff who may be experiencing emotional issues as a result of the involvement with families.
Responsibility for day to day supervision of band 6 reviewers, family liaison officers and project support staff.
Organise and provide case management and clinical supervision for individuals and any group supervision as required.
