Job summary

GP Practice in Ashington and Newbiggin

Care Co-ordinator

The successful candidate will have experience of working in health, social care or other support roles which are in direct contact with people, families or carers.

They will have excellent interpersonal and communication skills; good organisation and administrative skills, including minute taking and writing.

Experience of working in a primary care environment would be advantageous but is not necessary.

Main duties of the job

The aim is to help people improve their quality of life, avoid unplanned admissions and reduce the effects of long-term conditions on their well-being.

Care Coordinators will play an important role within the Primary Care Network to reduce health inequalities and support meeting both PCN and Practice targets. They will work closely with the Practice and PCN staff to identify, engage with and proactively coordinate personalised care and support planning for the most vulnerable in our community, including the frail/elderly, people living in care homes, people with mental illness and those with long-term health conditions.

We work as part of Wansbeck Primary Care Network with 4 other practice including Bedlingtonshire, Gables and Guidepost covering c. 47,000. The Network has a variety of Additional Roles including Community Link Workers, Mental Health Practitioners, First Contact Physios, Health & Well Being Coaches, Dietician and Pharmacists.

Qualifications

Essential

  • Functional Literacy and Numeracy level 2/GCSE Grade A-C, including Maths and English Language.

Desirable

  • NVQ level 3 in health and social care related discipline.

Experience

Essential

  • Experience of working in health, social care or other support roles which are in direct contact with people, families or carers.
  • Demonstrable and proven organisational capability in a complex environment.
  • Working in a confidential environment.
  • Trained in the use of Microsoft Office including Word and Excel.
  • Awareness of Data Protection Act and confidentiality.

  • Knowledge of the Primary Care environment.
  • Working in a GP Practice.
  • GP Clinical systems.
  • Experience of working with people who may face health inequalities
