Practice Nurse

The ideal candidate will be an experienced and clinically skilled Practice Nurse able to provide a range of Nursing services, including Chronic Disease Management as well as core Practice Nursing duties.

The role offers excellent opportunities for training, support, mentorship and personal development. There is a minimum requirement of two years post registration experience.

Main duties of the job

To provide a comprehensive range of nursing services to the whole Practice population, delivering a cost-effective service, maximising access for our patients and Practice achievement in QOF, Enhanced Services and other new contractual incentives as they arise.

The focus of the role encompasses the delivery of evidence-based practice for patients presenting with a long-term condition, the provision of preventative health care to the Practice population and working autonomously within a specialist clinical area.

The post holder will also provide clinical supervision and support professional development for more junior professional and non qualified colleagues within the Practice Nursing Team.
