Job summary
Sedgefield North Primary Care Network is an innovative,
dynamic PCN comprising 5 local GP Practices based in Spennymoor, Ferryhill
& Chilton, Sedgefield and West Cornforth with a population of over 57,000
patients. We are continually
seeking new ways to support and improve local healthcare services for our
patient population.
An exciting opportunity has arisen for two Cancer
Facilitators to join the PCN workforce to help drive improvements in cancer
outcomes for patients through the delivery of early diagnosis of cancer in the
PCN, with a specific focus on improving the uptake of cancer screening by
patients.
The Cancer Care Facilitators will be employed by Bishops
Close Medical Group on behalf of Sedgefield North PCN, aligned to specific
practices, but to work flexibly as required.
You will be joining the growing PCN multi-disciplinary team, with a wide
range of roles including Care Co-ordinators, Health & Wellbeing Coaches,
Social Prescribing Link Workers, Clinical Pharmacists, Pharmacy Technicians,
Trainee Nursing Associates, First Contact Physiotherapists and Primary Mental
Health Nurses.
We are committed to the ongoing development of all PCN
roles, which includes mandatory and developmental training for all our staff.
Team members are encouraged to highlight any training they feel would benefit
them in the achievement of their role.

Practices based in Spennymoor, Ferryhill & Chilton, Sedgefield, and West
Cornforth with a population of over 57,000 patients. The practices have been working together as
a PCN since 2019, in partnership with local community health providers,
mental health services, care homes and community pharmacies. We are
continually seeking new ways to support and improve local healthcare
services for our patient population, by working collaboratively with other
agencies and the local population. Through investment in new roles the PCN is
expanding the range of services that are available to support practices
deliver a high-quality care experience to their patients. The PCN now has a multi-disciplinary
workforce which includes SPLWs, Care Co-ordinators, Health & Well-being
Coaches, Trainee Nursing Associates, Clinical Pharmacists, Pharmacy
Technicians, First Contact Physiotherapists, Advanced Nurse Practitioners and
Primary Mental Health Nurses.

practices and the PCN with campaigns that could encourage further uptake of
screening
- Work
with local organisations in supporting already established campaigns.
Active
engagement with all cancer types including those that dont currently have a
national screening programme
- Work
with practices to achieve attendances for 2 week wait referrals in prostate
cancer and other less commonly seen cancers.
Practice

- Offer
support and training to engage practice staff in cancer screening uptake and
early diagnosis
- Establish
a practice network of cancer champions who can work alongside the PCN Cancer
Facilitators.

- Educated to GCSE Standard (Grades A-C) or Diploma/HNC Level
Desirable
- NVQ or other qualifications in health and social care
- NVQ Level 3 Business Administration (or relevant experience)
- ECDL or equivalent

- Relevant experience of working within the NHS, charitable or third sector at Agenda for Change Band 3 or 4, or equivalent
