Job summary
An exciting opportunity has arisen within our Continuing Health Care Teams across Greater Manchester. We are looking for highly motivated and experienced RMNs, RGNs OR Allied Health Professionals who can work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC). You will undertake comprehensive person-centred assessments so CHC and FNC eligibility can be determined.
Roles involve assessing patients for CHC, holding a caseload, and reviewing patients, as necessary.
Main duties of the job
The successful post holders will have the opportunity to work within a successful, progressive Integrated Care organisation and make a positive contribution to the lives of people in Greater Manchester.
There are six locality place bases with current vacancies: Manchester, Oldham, Salford, Stockport, Tameside and Trafford. NHS GM offer fully flexible roles open to all Allied Health Professionals and Registered Nurses.
The post holders will play a key role in supporting the All-Age Continuing Health Care Service across Greater Manchester.
The post holders will assist and support in the facilitation of multi-disciplinary assessments and patient reviews in line with standard operating policy and procedures, ensuring the National Framework for Continuing Healthcare and Funded-Nursing Care is implemented and complied with, according to the principles and processes.
Our hours of work are Monday - Friday, working flexibly but ensuring core hours are met within the locality. There is an opportunity for Hybrid working which is a combination of face to face, office and home working.

Hospitals, GPs, community services, voluntary services and others have come together to form 'provider collaboratives' within all 3 levels, helping to join care and help people live well across our ten boroughs.
