Job summary

1) To work proactively across organisational boundaries to improve health outcomes and the way services are delivered,

o to improve access to appropriate care

o to reduce health inequalities and gaps in service provision

o to prioritise, lead and direct programmes of work which are designed to improve health outcomes for Bolton people at population level

o to provide clinical oversight, and leadership to the wide-ranging programme of work

o to regularly engage and collaborate with individual GPs, and their practice staff

2) Specifically, to provide clinical leadership for Primary Care Transformation, Interface, Prescribing and Quality within the Bolton Locality by working across interfaces with:

o NHS GM, including NHS GM Medical and Nursing Directorates, SCNs and NHS GM System Boards

o Locality partners, including Bolton FT, Community Services, Council and VCSE

o Individual GPs, Practice Managers and their Practice teams

3) To support delivery of the themes and priorities of the NHS GM Strategic Plan and national policies e.g. Bolton Locality Plan, BHSCP Strategic Plan, Bolton Quality Contract

Main duties of the job

Working with other CCPL leads to deliver CCCP Leadership Principles. Lead the development and delivery of services, working with managerial, political and CCPL leaders across the ICS and locality, supporting the delivery of quadruple aim whilst keeping quality and safety at the heart of everything we do. Lead improvement in outcomes for population health and wellbeing through a lens of prevention and inequality. Put the patient at the centre, build key relationships with patients, service users/carers. Be active in the clinical and professional workforce.Engage with wider clinical networks, PCNs/SCNs across health and social care system. Represent the locality and CCPL voice in GM. Act as a CCPL leader, not on behalf their employer or professional group. Promote clinical leadership reflective of our population and support in identification/recruitment of a pipeline of future CCPL leaders. Support the development of care pathways and patient information. Support and link with the clinical education programmes. Link to the locality CCPL Forum and be a member of the Locality GP Board. Good knowledge of the Bolton locality. Build excellent working relationships with Bolton GPs. Proactively focus on addressing inequalities with the use of the CORE20+5 model. Support the AMD to deliver the requirements of the Bolton Locality Plan and NHS GM ICB requirements.

  • Formal Management Qualification and / or proven and significant leadership experience.

Knowledge

  • Be working in a Clinical Role within the Place
  • Detailed knowledge and understanding of NHS Health and Care services at a National, Regional and local level

Desirable

  • Experience of working at a Senior level within the NHS and considerable senior experience of leading in integrated health and care settings.
  • Significant experience of leading and delivering complex change and strategy development programmes in a politically sensitive and complex environment.
  • Experience and track record of managing very senior relationships across multiple partner organisations
