Job summary

An exciting opportunity has arisen within our Continuing Health Care Teams across Greater Manchester. We are looking for highly motivated and experienced RMNs, RGNs OR Allied Health Professionals who can work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC). You will undertake comprehensive person-centred assessments so CHC and FNC eligibility can be determined.

We have vacancies across various localities in Manchester including:

  • Stockport
  • Trafford
  • Wigan

Roles involve assessing patients for CHC, holding a caseload, and reviewing patients, as necessary.

Interviews will be held on Wednesday 20th March 2024 at Manchester City Centre Location

Main duties of the job

  • The post holder will be responsible for undertaking Specialist Health Needs assessments for individuals who are undergoing assessment for, or who are already eligible for NHS Continuing Healthcare and NHS Funded Nursing Care.
  • The post holder will be required to work with the individual and/or their representative, and to liaise with private providers, partner organisations, along with other health and social care professionals to ensure that all assessments are person centred.
  • The post holder will be required to work flexibly to support the Individualised Commissioning team by participating in the 'Duty' rota, supporting with any urgent referrals or any safeguarding investigations.

Hospitals, GPs, community services, voluntary services and others have come together to form 'provider collaboratives' within all 3 levels, helping to join care and help people live well across our ten boroughs.

The role sits in the Continuing Healthcare & Individualised Commissioning team which is part of a wider Directorate led by the Associate Director of Nursing, Quality and Safeguarding.

Experience

  • Significant nursing experience at band 5 or equivalent experience Evidence of post qualifying and continuing professional development Experience of working as a Registered Nurse with patients with multiple/complex needs Responsibility for a defined caseload which includes assessment, planning, implementation and evaluation of nursing care Experience in working within a multi-disciplinary team setting Experience of clinical supervision and reflective practice discussions

Desirable

  • Experience of working with Older People with Mental Health difficulties

  • Knowledge and understanding of current national policy/framework guidance relating to NHS Continuing Healthcare and NHS Funded care. Awareness of current legislation relating to health and social care Understanding of the current issues impacting on the NHS Confidence to manage complex multi-disciplinary.
