Job summary

An exciting opportunity has arisen within our Continuing Health Care Teams across Greater Manchester. We are looking for highly motivated and experienced RMNs, RGNs OR Allied Health Professionals who can work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC). You will undertake comprehensive person-centred assessments so CHC and FNC eligibility can be determined.

Roles involve assessing patients for CHC, holding a caseload, and reviewing patients, as necessary.

Main duties of the job

The successful post holders will have the opportunity to work within a successful, progressive Integrated Care organisation and make a positive contribution to the lives of people in Greater Manchester.

The three place bases with current vacancies are, Manchester, Stockport, and Trafford. NHS GM offer fully flexible roles open to all Allied Health Professionals and Registered Nurses.

The post holders will play a key role in supporting the All Age Continuing Health Care Service across Greater Manchester.

Working within the remit of the National Framework for Continuing Healthcare, the post holders will deliver an effective and competent level of service, consistently delivering a client-focused service.

The post holders will assist and support in the facilitation of multi-disciplinary assessments and patient reviews in line with standard operating policy and procedures, ensuring the National Framework for Continuing Healthcare and Funded-Nursing Care is implemented and complied with, according to the principles and processes.

The role sits in the Continuing Healthcare & Individualised Commissioning team which is part of a wider Directorate led by the Associate Director of Nursing, Quality and Safeguarding.

Job responsibilities

The post holder will take a lead role in the identification of suitable care providers and will be responsible for ongoing review and reassessment of patients (case management) of commissioned packages of care. Case management will be with a view to determining continued eligibility for CHC and to ensure that the package of care continues to meet assessed needs and is of a suitable quality.

The post holder will be expected to work with a wide range of NHS and non-NHS health providers, including General Practice, and with the Local Authority across the borough.

  • Coordinating the process of determination of eligibility for NHS funded Continuing Healthcare within the parameters of the National Framework for NHS funded Care
  • Deputise for Lead Nurse in issues around Continuing Healthcare, NHS Funded Care and Complex care.
  • Represent NHS GM Integrated Care at multi agency meetings.
  • Liaise with other professionals, providers and key stakeholders where required.
  • Liaise with providers over concerns raised at review and instigate safeguarding procedures where necessary.
  • Liaise with patients families, and carers with regard to CHC and review process.
  • Manage a caseload of patients who have been assessed as eligible for NHS Continuing Health care or a jointly funded package of individually commissioned support.
