Job summary
NHS Greater Manchester ICB Bury Locality operate a Continuing Healthcare / Complex Case service on behalf of the registered population Bury. The post holder will be responsible for coordinating the process of determination of eligibility for NHS funded Continuing Healthcare within the parameters of the National Framework for NHS funded Continuing Healthcare for patients living in the Bury locality.
The post holder will take a lead role in the identification of suitable care providers and will be responsible for ongoing review and reassessment of patients (case management) of commissioned packages of care. Case management will be with a view to determining continued eligibility for CHC and to ensure that the package of care continues to meet assessed needs and is a suitable quality.
The post holder will be expected to work with a wide range of NHS and non-NHS health providers, including General Practice, and with the Local, Authority across the borough.
Main duties of the job
- Coordinating the process of determination of eligibility for NHS funded Continuing Healthcare within the parameters of the National Framework for NHS funded Continuing
- Deputise for Lead Nurse in issues around Continuing Healthcare, NHS Funded Care and Complex care.
- Represent GM ICB- Bury at multi agency meetings.
- Liaise with other professionals, providers and key stakeholders where required
- Liaise with providers over concerns raised at review and instigate safeguarding procedures where necessary.
- Liaise with patients' families, and carers with regard to CHC and review process
- Manage caseload of patients who have been assessed as eligible for NHS Continuing Health care or a jointly funded package of individually commissioned support
