Primary Care Digital Facilitator Role

We are looking for someone who is dynamic and conscientious to join our Digital Facilitation Team for primary care, as part of the Greater Manchester digital first primary care program. The digital first primary care program will work across all 10 Greater Manchester localities.This role holds a significant amount of personal responsibility and accountability: You will need to be someone who can self-motivate, problem-solve, and manage your own workload and priorities. You will possess excellent interpersonal and communication skills to be able to build relationships with the GP Practices you work with and support with other, Greater Manchester and general practice digital first programs and projects. You will do this by working with people, building relationships, and directly supporting staff working in General Practice.

You will be joining a new team of primary care digital facilitators and will receive supportive training in helping you develop service improvement and digital transformation skills and primary care IT system training.

We are interested in applicants based anywhere in Greater Manchester, so please include your 1st and 2nd choice locations in the additional information part of your application form, from the bases located in one of the following 6; Bury, Trafford, Oldham, Central Manchester, Tameside and Glossop (T&G), Heywood, Middleton and Rochdale (HMR).

Main duties of the job

To co-ordinate the use of GP clinical systems and digital technologies within primary care, in order to support improvements to the health of the local population and to support the general management of patients and practices. This will involve supporting neighbourhoods, standardising data recording and working processes as well as facilitating training and behaviour change regarding Information Management and Technology within the neighbourhoods. The primary care Digital Facilitator will work with neighbourhood managers to deliver targets.

There would be an expectation for you attend the office as required and travel in the area of your locality, including across into other areas of Greater Manchester as required and dependent on the programme of work

The successful candidates will be enrolled on to a mixture of face to face training and then, quarterly training will mainly be delivered via Microsoft teams to support skills improvement.

About us

The digital facilitators for primary care

The roles are unique and new for Greater Manchester aimed to improve patients access to primary care by optimising digital.

The facilitators will be recruited to work mostly in one of six of Greater Manchester's 10 localities and will also work with other digital facilitators to carry out work for Greater Manchester in line with the formation of our integrated care system.

Day to day a digital facilitator will be working with GP practices in a locality with the support of the locality IT team and they will report to a senior digital facilitator leader who will be responsible for the digital facilitators across five of the localities, for support and line management.

In addition, the digital facilitators will be enrolled into our new digital first academy where they will be able to receive training and support with skills to enable them to be effective facilitators and develop new transferable skills that can be captured and used in this role and help the facilitators in their career journey in the NHS.
