Job summary
Do you have an interest in nutrition and the promotion of health?We are looking for an enthusiastic individual to be part of our team to support patients in general practice to reduce their risk of getting diabetes. The post holder will work within a designated primary care cluster within Swansea Bay University Health Board area. The post is permanent, full time (working 37.5 hours per week).
The project is based in GP Practices and other community sites therefore an ability to travel between sites in a timely manner is essential for this role.
Main duties of the job
You will receive nutrition and communication skills training to deliver a brief intervention with patients identified as being at risk of having diabetes. This will involve having diet and lifestyle conversations and signposting to local services to support a reduction in the risk of development of diabetes. The role will include the collection of a minimum data set and support of the development and distribution of written resources.
The successful candidate will be expected to undertake appropriate training to use the software, deliver the brief intervention and also to develop excellent communication and behaviour change skills which will enable them to support and motivate patients to make positive lifestyle changes.
You will join a small supportive team under the leadership of the Diabetes Prevention Dietitian and you will have links to a network across Wales. You will work together with the Dietitian, GPs and wider practice team in the delivery of care to the practice population.
Experience of working in a healthcare setting or primary care and good IT skills will also be an essential requirement of the post.
We offer departmental CPD time to support your development and are committed to the development of teaching and presentation skills, evidenced based practice, research and clinical audit. You will be supported in attending relevant networking meetings and clinical focus groups.
