Job summary

An exciting opportunity has arisen for an enthusiastic, suitably qualified Community Nurse to join our Health Visiting Team in Swansea Bay University Health Board.

The successful applicant will carry out duties delegated by health visitors in a range of settings including persons own home, clinics and groups. The community nurse will be part of a dynamic health team to provide advice and support to children and their significant others building resilience and providing a positive framework in the crucial early years. The health visiting team is an innovative team that seeks to promote evidence-based research and practice in order to view the family holistically and promote children and their significant others to reach their full potential. Community nurses within the health visitor team are highly trained with additional skills to meet the additional needs of children under their care and a full preceptorship and educational package will be awarded and supported.

Main duties of the job

The main duties of the role of the community nurse are to participate in the assessing, planning, delivering and evaluating care for patients/clients as part of a multi-disciplinary team.The post holder will be a member of the health visiting team, carrying out all duties under the supervision of the case holding Health Visitor. These duties will include a combination of working with children and their significant others at home or in a group environment. (Clinical Responsibilities).The post holder will liaise with and work in partnership with members of the wider Health Visiting Team and voluntary and statutory agencies as appropriate. (Communication Skills). The community Nurse will contribute effectively to delegated administration and clerical duties. (Managerial Skills)
