Merthyr Tydfil Community Learning Disability Team has a vacancy for a Specialist Band 6 Physiotherapist.

The Learning Disability Service of the Primary Care, Community + Therapies Group of Swansea Bay University Health Board provides a service to adults with a Learning Disability across 3 Health Boards and 7 local authorities. Cwm Taf Morgannwg Health Board commission their Learning Disability services from Swansea Bay Health Board.

We are seeking a Specialist Physiotherapist to join the well established Community Learning Disability Team in Merthyr Tydfil. The team is comprised of Community Nursing, Occupational Therapy, Speech and Language Therapy, Psychology and Psychiatry and works in close partnership with local authority colleagues and partnership organisations.

The team is committed to the delivery of user focused services to improve the health and well-being of the population.

Main duties of the job

To work as an autonomous practitioner in accordance with the standards and codes of practice of the Chartered Society of Physiotherapy (CSP), the Health Care Professions Council (HCPC), and within the Clinical Governance framework and Welsh Government policy.

To be a member of the multidisciplinary Community Learning Disability Team (CLDT) comprising health and social care professionals.

To provide specialist physiotherapy services through a combination of assessment, diagnosis, formulation of treatment programmes, intervention and teaching that aim to improve, maximise and maintain the physical ability and minimise the disability of people with a learning disability.

To be responsible for a defined caseload as allocated, using evidence based practice

and client centred principles to assess, plan, implement and evaluate interventions that aim to achieve best outcomes for the client.

To provide advice to carers, team members, partnership agencies in supporting clients and implementing physiotherapy programmes in the client's home and community environment.

To supervise staff and delegate duties.

To supervise Physiotherapy students on clinical placement.

To develop knowledge and skills that meet the competency framework through a range of study.

Service provision is over a large geographical area requiring lone working and daily travel between the base and treatment locations.
