Job summary

We are looking for an enthusiastic and caring individual to work as part of our team in Older Persons Services in Derwen Ward.

Derwen Ward is set in Ysbryd Y Coed and part of 3 separate Wards: Derwen, Onnen and Celyn. Derwen is a 20-bedded Male Dementia Assessment/admissions Inpatient Ward, Onnen Ward is a 18-bedded Female Dementia Assessment/admissions Inpatient Ward and Celyn is functional Older Persons Mental Health assessment /admissions Ward all three are based in Ysbryd Y Coed which is based in Cefn Coed Hospital.

Main duties of the job

To contribute to the development of those accessing continuing healthcare ensuring a holistic delivery of care and services which are coherent and consistently applied.

The post holder will assist in the promotion of equality for all clients through anti-discriminatory practice, supporting clients' rights, choices, personal beliefs and identity and by supporting clients with challenging behaviour and complex health needs through effective communication.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
