Crisis Intervention Worker / Mental Health Practitioner

Job summary

The mental health practitioner will support people experiencing a mental health crisis providing mental health care, advice, support and treatment where appropriate. The service is for people who are over the age of 18 is Swansea prison based the post holder will co work with In-reach and Core mental health team where appropriate.

Main duties of the job

The mental health practitioner will work to improve access to mental health care for people with mild to moderate or stable severe mental health problems and or those in mental health crisis by delivering holistic assessment, and evidence based care to clients within the context of a multi-disciplinary multi-agency Prison healthcare setting. This will include liaison with Professional leads, managers, multi-disciplinary team members including Prison officers, Prison Care mental Health Team and Inreach service, Safer Custody Team, network of external agencies and local community groups Coordinate own work load, accept email and phone referral from healthcare team, officer team.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
