Job summary

An exciting opportunity has arisen for enthusiastic Health Visitors to join the Swansea Flying Start Health Visiting Team within Swansea. We have a full time and part-time position available.

The post holder will be responsible for delivering the Flying Start enhanced health visiting programme for a defined caseload of children, families and communities to support children reach their full potential.

The Health Visitor will be expected to work in partnership with other professionals and agencies to address health needs - demonstrating excellent communication skills whilst working both autonomously and as part of a team.

The post holder will be committed to developing a high degree of standards of care and actively seek and promote professional working relationships with the wider Flying Start Team, and statutory and voluntary practitioners. The individual will be passionate about promoting public health, promoting specialist knowledge and advice to families enabling them to lead healthy lifestyles, prevent illness and to give their children the best possible start in life.

In order to apply you will need to possess a Health Visiting qualification and be registered on the appropriate part of the NMC register.

Main duties of the job

This role is designed to meet the needs of the local population with the overarching aims of increasing family resilience, improving population health outcomes and reducing health inequalities.

The post holder will be providing the All Wales Flying Start Health Visiting Programme to children and families in a defined area of Swansea.

The post holder will be responsible for the assessment of health needs and the development, implementation and evaluation of programmes of care to meet the needs of the child and their family following the Flying Start Healthy Child Wales Programme.

The post holder will work with families and young children in their homes and other venues within the community to provide advice, support and interventions to help parents make decisions which positively affect their family's future health and wellbeing.

In order to provide a seamless service for children, families and communities, the health visitor will actively work in a multi-agency environment and actively promote effective working relationships and communications with colleagues, professionals and statutory agencies.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
