Job summary

An exciting opportunity has arisen for two band 6 Acute Liaison Nurse posts within secondary care.

The ideal candidate will have excellent communication skills and be able to demonstrate effective working with multiagency colleagues and advocate for the needs of patients with learning disability.

The primary focus of the role will be to support acute hospital staff in the effective delivery of person centered care to patients with a learning disability. As a first point of contact the post holder will provide expert advice, professional support, guidance and education to the multi-professional teams, partner agencies, patients and carers. You must have the ability to work both as part of a team and autonomously, have excellent interpersonal skills and be able to prioritise and manage your own workload. You must be committed to advocating for the rights of vulnerable people and act as a role model at all times.

Main duties of the job

The post is for a Band 6 Acute Liaison Learning Disability Nurse who will be based in Prince Charles / Royal Glamorgan hospital to facilitate reasonable adjustments to enable people to access health care. The role will involve training up staff within the hospital in the use of the Hospital Care Bundle, communicating with and assessing people who have a Learning Disability, what making reasonable adjustments entails, supporting the person and their carer's and liaising with the wider MDT in the community.

The role will require collaboration with colleagues in neighbouring Health Boards to ensure audit and data collection is comparable and links to Improving Lives driver of better outcomes for people accessing hospital.

The post holder will be line managed from the Community Team where they will also receive clinical supervision from the Lead nurse, however they will be based within the hospital environment.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
