Are you an experienced Primary Care or Community Nurse looking for a role in General Practice? We have an exciting opportunity available for a Practice Nurse role within our Health Board Managed Practice which is based across two clinical sites, Cymmer and Cwmavon.
The successful applicant will be responsible for providing nursing services to patients accessing SBUHB current and future managed practices and any branch surgeries. We are seeking a dynamic and innovative individual with a passion to continuously improve patient care and service delivery.
The post is being advertised for 16 hours per week, working days will be between Monday and Friday. Experience in Primary Care or Community Nursing is ideally required.

Main duties of the job
Main duties of the role are as follows:-
- Provide nursing services to patients accessing SBUHB current and future managed practices and any branch surgeries.
- Delivery of evidence-based practice for patients presenting with chronic conditions, and the provision of preventative health care to the practice population.
- To work as an autonomous nurse within current competencies, demonstrating critical thinking and skills in clinical decision-making to support the management of patients.
- Support new models of care within the managed practice, including telephone first concepts and the alternative workforce roles.
- To deputise for nursing colleagues during any periods of absence.
- Support the Sister/Charge Nurse and Senior Nurse in delivering and supporting effective care to patients within a managed practice.
