Job summary
To act as an expert specialist social work resource for Ty Olwen palliative care, providing support and advice to all referred patients, family/significant others and or carers.
The service offered by the department aims to contribute to the multidisciplinary, holistic approach that is concerned with the physical, psychological, spiritual, emotional and social wellbeing of the patient and their family/and or carers. Issues of living with life limiting diseases, impending death, loss and grief are a feature of this work.
The postholder will proactively work as part of the multidisciplinary team, advocating on patients and families' behalf to contribute to a seamless specialist palliative care service, facilitating timely complex discharge management and supporting patients in advance and future care planning, including achieving their preferred place of care at the end of life.
With the other multi-disciplinary professional team surrounding the person and those important to them, the social worker ensures that services and interventions take account of the whole person as well as their family, whatever that means for them, seeing whole people living within whole families rather than as an individual with a set of specific problems to solve.
The social worker will understand the connections of life and support people as they come to terms with what is happening to them or someone close to them, and support them to continue to live as they choose for as long as they can.
Main duties of the job

This post will cover in patients, but at times outreach to the community and other areas may be required as part of assessment and planning care.Actively contributes to the development, delivery and evaluation of the post, and service delivery.
To undertake continuing holistic assessment of patient and carer needs, providing and managing social work care for specialist palliative care patients and their families.
Promotes people's equality, diversity and rights.
Liaises and shares specialist knowledge on the social needs of specialist palliative care patients with both statutory and non-statutory agencies within both the hospital and the community.
Analyses, interprets and reports factual data and information.Develops programmes of social care/care packages; co-ordinates and develops care packages or support services, advises carers and families.
