Job summary
We are delighted to advertise this brand new cross-sector role for an early career pharmacist. You will work within the renal pharmacy team in Morriston Hospital for 2 days a week and in Ty'r Felin Surgery for 2 days a week. During this HEIW Post-Registration Foundation Pharmacist (PRFP) training programme you will be supported by your educational and clinical supervisors to reach your goals, including independent prescribing. You will receive 1 day a week protected study time.
Department of Nephrology
You will work under the guidance of experienced renal pharmacy professionals and will be responsible for clinical and logistical aspects of medicines optimisation for people with kidney disease, including inpatients in Morriston Hospital and outpatients requiring haemodialysis (HD) and kidney transplant recipients across South West Wales.
Ty'r Felin Surgery
You will be supported by a Clinical Pharmacist and General Practitioner to undertake patient consultations, manage chronic conditions like diabetes and Chronic Kidney Disease (CKD), manage acute medication requests and contribute to the education and training of the Practice's MDT.
You will explore how specialists and generalists can work together more effectively, and how you, as an individual working across traditional healthcare boundaries can develop new and innovative ways of working.
You should have a passion for improvement and patient-focused care, and be able to work well as part of a multidisciplinary team.
Main duties of the job
FIXED TERM/SECONDMENT FOR 2 YEARS DUE TO TRAINING POST
HEIW Post-Registration Foundation Pharmacist programme
- As a PRFP learner you will be required to sign an agreement with HEIW, the renal pharmacy team and Ty'r Fein Surgery that will explain the roles and responsibilities of all parties
- Under the PRFT Programme you will be provided with a dedicated practice supervisor, a Designated Prescribing Practitioner (DPP) and protected learning time to achieve your goals; including an Independent Prescribing (IP) award should the IP module be undertaken in the second year of the programme
Renal Pharmacy Department
- Participate in the multidisciplinary review of renal patients (both inpatients and outpatients) sharing specialist knowledge and expertise with the MDT
- Participate in renal pharmacy services including the clinical and final accuracy checking of prescriptions, inpatient/outpatient orders and discharge prescriptions
- Maintain accurate medication records
Ty'r Felin Surgery
- Undertake patient consultations
- Chronic disease management including Chronic Kidney Disease
- General medication and hospital discharge reviews
- Acute medication requests
- Contribute to education and training of the Pharmacy team and wider MDT
- Advise the clinical team RE medication management as appropriate
- Ensure appropriate documentation and adherence with recommended monitoring
- Identify patients with poor compliance or high risk use for clinical review
- Contribute to enhanced service provision
