Job summary
This is an exciting opportunity to join part of the Forensic Gate keeping and Case management Team in Caswell Clinic Medium Secure Services. The role will entail Gate Keeping and the management of clinical cases with in Medium Secure independent/NHS sector placements and High Secure Placements. The post holder will exercise a high level of judgment in monitoring this group of patients with complex needs. This will include case reviews, attendance at CTP reviews when required and will provide clinical assurance as to the appropriateness of placements, development of care pathways and ensure administration of step down process.
The post holder will be required to manage his/her own workloads, set priorities, milestones and deadlines, ensuring they are meeting the team's performance requirements. This will require the post holder travelling to various facilities across England and Wales which when required will include overnight stays.
The post holder will be responsible for the case management of a specialised cohort of patients requiring either medium secure or high secure care. The patient group will be both male and female with a diagnosis of mental health and or learning disabilities. Previous experience in relation to the commissioning process in both England and Wales would be desirable.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
Main duties of the job
The post holder will be responsible:
- Undertake proactive, case management (or reactive to situations that have arisen) of those individuals placed in the Medium independent care sector, NHS and High Secure.
- Undertake formal clinical case reviews of patient care within their current placement and complete a case review report.
- Providing support and professional advice to the provider, care coordinator and Quality Assurance and Improvement Team.
- Producing written information resources, such as reports and letters including undertaking specific projects as requested by the service.
- Ensuring all information, electronic, written and verbal is appropriately recorded and stored in accordance with information governance policies.
- Supporting the review and reporting of Adult Safeguarding issues.
- Ensuring work is available to deadline when necessary.
- Supporting the reporting and investigating of independent sector provider issues.
- Ensuring services are delivering the Quality Assurance requirements of their service.
- Supporting the Lead Nurse and Directorate Manager in other functions that remain.
- Taking an active role in service developments and working in the capacity of Senior Nurse with in the Caswell Clinic, providing clinical supervision and mentorship/ role modelling to the wider nursing workforce.
