The Cardiology department are looking for an enthusiastic Senior Clinical fellow to join the department.Morriston Hospital currently performs around 300-350 TAVIs per annum using the Edwards, Navitor and Allegra systems with the vast majority of cases performed under local anaesthetic via the trans femoral approach. Surgical access TAVI is supported by our vascular and cardiothoracic surgical teams. 5 Consultant cardiologists deliver the procedural service, Professor Dave Smith, (TAVI lead), Professor Alex Chase (TAVI/PCI), Dr Ayush Khurana (TAVI/PCI), Dr Dan Obaid (TAVI/PCI) and Dr Ahmed Hailan (TAVI/ PCI). In addition to TAVI there is a growing structural programme including paravalvular leak closure, and depending on time commitments there may be opportunities to be involved in these cases. The TAVI team are a designated European Centre of Excellence for Abbott and an Edwards benchmark accredited centre, regularly undertaking teaching and training for clinical teams from the UK and internationally.
Main duties of the job
The TAVI service at Morriston Hospital has developed significantly since its inception in 2008. Expected annual activity for 2023 includes 350 TAVI procedures, with use of three different TAVI valve systems. We also perform paravalvular closure procedures and transcatheter mitral valve implant procedures. The TAVI fellow is the sole senior trainee in TAVI and is actively encouraged to participate in all of these procedures.
The fellow has the opportunity to gain extensive experience in patient assessment and participate in the weekly aortic stenosis clinic during which patients are jointly reviewed by a cardiac surgeon and a cardiologist. The fellow will also participate in the weekly structural heart MDT as well as TAVI procedure planning meetings. Full training will also be given in computed tomography (CT) analysis to facilitate procedural planning.
The Cardiac Centre as a whole undertakes approximately 2300 angiograms, 1700 percutaneous interventions with a busy primary PCI service, 550 open-heart operations annually. A newly developed Cardiac MRI service further compliments the service.
You will be expected to deputise for absent colleagues and in exceptional, unforeseen circumstances may have to perform additional duties to ensure the safe running of the department. Where such duties take place outside your normal contracted duties, you will receive either an equivalent off duty period or will be remunerated appropriately.
About us
Morriston Hospital Cardiac Centre
The centre currently provides Tertiary Cardiac Services for the entire population of mid and West Wales, and it also provides CCU and secondary care facilities for Swansea. Tertiary Cardiac Services commenced at Morriston Hospital in 1997. The centre combines cardiology and cardiothoracic surgery, which have a very close working relationship. The units catchment area is large and diverse, some of it urban and industrialised, and some very rural and remote, from Bridgend in the South East to Machynlleth in the North West.
The Centre has a purpose-built three-story building, which is part of the main hospital. The lower ground floor is composed of an outpatient consultation area and non-invasive cardiology suite including facilities for Holter monitoring, event recorders, exercise testing and echocardiography.
The main ward include three cardiac wards, 8 bed Coronary Care Unit, a 12 bedded short stay unit, X-ray screening room for emergency procedures, a new 16 bedded cardiac ITU and 8 bed surgical HDU. There is also an adjacent office area for consultants and secretaries with a well-equipped seminar room, complete with teleconferencing facilities.
The second floor contains two cardiac theatres and three catheter labs and a pacing suite.

The post will be based at the Regional Cardiac Centre at Morriston Hospital. It will be suitable for an applicant who holds a certificate of completion of training (CCT) or National Training Number and is within one year of completion of their training programme, or equivalent.
Applicants will be expected to be proficient in diagnostic coronary angiography, and basic and advanced coronary angioplasty techniques. No prior experience of performing TAVI procedures is required but is desirable.
The training fellowship will last for 12 months and the successful applicant will have at least 7 fixed sessions allocated to TAVI procedures, and aortic stenosis outpatient clinic. The remainder of the timetable will be committed to cardiac CT analysis and reporting as well as audit and research interests the successful candidate might wish to pursue. There is no additional clinic commitment or bleep holding duties in hours. For applicants wishing to maintain their skills and experience in coronary intervention there will be the opportunity to participate in urgent and emergency PCI cases.
The candidate will be encouraged to participate regularly in the breakfast meetings and MDT, presenting unusual and educational cases to the department and engaging in difficult clinical decisions.
All procedures will be performed under the supervision of the consultant TAVI cardiologists.
On-call
The successful applicant will participate in the 1:10 partial shift on-call rota. Duties are resident but confined to the cardiac centre and receiving urgent cardiology referrals within the Trust or neighbouring hospitals. There are no general medical on-call duties. The post is New Deal and European Working Time Directive compliant.
