Job summary
Children's Community Nurse -Band 6.
The successful candidate will provide leadership, professional advice and care for the Community Children's Service in SBUHB. The post holder will have responsibility for management of a designated caseload delegating responsibilities and tasks as required.
They will undertake specialist nursing assessments and plan, deliver, and evaluate care for children with acute intervention, complex chronic illness and continuing care needs. The post holder will work in partnership with the appropriate multi-disciplinary agencies to provide skilled nursing care to children and young people.
The post holder will be expected to work across Swansea and Neath Port Talbot areas according to service needs.
Main duties of the job
To facilitate the provision of effective, efficient and appropriate high quality care to Community Children's Nursing Service ensuring the highest standard of nursing care is provided, monitored and evaluated.
To support the Community Children's Nursing Team in assessing, planning, implementing and valuating the holistic nursing and health care needs of children and young people with a range of conditions from acute illness to complex, life limiting and end of life care.
