About us

Swansea Bay UHB Urgent Primary Care Service is offering an exciting opportunity for highly motivated, enthusiastic and experienced GPs to provide Urgent Primary Care Services to Swansea Bay UHB residents and visitors, you will participate as a key member of the Urgent Primary Care teams in providing access, support and advice for all Urgent Primary Care presentations within the unscheduled model of care. You will be able to choose to work within a number of SBUHB unscheduled and emergency care services including: Urgent Primary Care, 111 Contact first, Acute GP Unit and ED GP sessions.

Job responsibilities

The post holder will be an employee of Swansea Bay University Health Board and will beable to choose to work within a number of SBUHB Unscheduled and Emergency Careservices:- Urgent Primary Care out of hours - undertaking triage of telephone calls at baseand remotely, treatment centre face to face appointments and home visits and anyadministration associated with patient care. The post holder will be expected todevelop and support the dissemination of best practice which would include thedelivery of staff training for other disciplines within the service. The post holder willassist the Lead Clinician in ensuring Clinical Governance is fully embedded in thework of the team.2- UPCC Urgent Primary Care Centre triaging and seeing patients redirected froma number of services (ED,111 Contact first , 111 clinical Hub ,WAST and GMSPrimary Care) with primary care conditions during in hours period- 111 Contact first Telephone triage of ED/MIU outcome calls from 111 to ensurepatient directed to most appropriate healthcare professional- AGPU Triaging calls from General Practice to care navigate to hospitaladmission/Ambulatory care/ Community services. The clinician will also assess andtreat patients on ambulatory care pathways.- ED GP sessions Triage and see patients with primary care problems presentingto ED
