Job summary

Swansea Bay University Health Board is transforming its acute medical services and applications are welcomed to appoint a Community based Geriatrician to work as part of the Acute Clinical Team. This is an exciting opportunity to be part of and shape care delivered closer to home but also develop pathways and working relationships with acute care colleagues. In line with strategic direction in Wales, primary and community services need to and will be at the forefront of service delivery

This community based post will help deliver a new model of integrated care for older people within Swansea Bay University Health Board building on established teams but also forge innovative working practices with our new Virtual wards across our 8 cluster footprint. You will join Dr Firdaus Adenwalla, a passionate clinician who has many years of experience in developing Acute Clinical Teams and a team of established Advanced Nurse Practitioners responsible for the provision of acute medical care for older people living with frailty in the community.

You will be based within the Acute Clinical Teams in Bon y Maen (Swansea) and Cimla Health and Social Care Centre (Neath Port Talbot).

Main duties of the job

The Acute Clinical Team delivers a broad range of acute medical care for older people living with frailty in the community. The team provides medical advice, assessment and management of a broad range of acute medical problems and acute exacerbations of chronic disease which would otherwise need hospital admission.

The Health Board is undergoing a major service change with significant investment in community services. £1.2 million has been invested in creating "virtual wards" to support the care of older patients in the community.

The Health Board and specifically this department has a strong commitment to undergraduate and post graduate education and close links to the Faculty of Health and Life Sciences in Swansea University.

Swansea is in South Wales which is a great place to live and work. The stunning Gower Peninsula is on your door step.Outdoor activities are a plenty with easy access to the Brecon Beacons National Park and unspoilt beaches of Pembrokeshire.
