Job summary

This post is a fixed term post joining a complement of seven Consultants, an Associate Specialist, three WTE Specialty Trainees, three Specialty Doctors (including this post) and a number of junior medical staff responsible for the provision of Specialist Palliative Medicine services to the Health Board.

The post provides a broad range of experience in Specialist Palliative care and the post holder would be supported to develop their career path in the speciality.

It is proposed that the post holder will further foster the development of multidisciplinary team working across the Health Board. The clinical focus of this post is as part of the community palliative care team in Swansea Bay or as mentioned as part of the Hospital advisory service.

The post is offered on the new amended Specialty Doctor contract on an optional whole time, 40 hours, working week (or pro rata) basis or part time bases and is subject to the terms and conditions of service governing the appointment of hospital medical and dental staff in Wales.

Main duties of the job

An exciting opportunity has arisen for a Speciality Doctor to join a friendly, ambitious and expanding department which provides a Specialist Palliative Care Service.

We are looking for an enthusiastic individualto join our multi-professional team in providing specialist inpatient, community (including domiciliary, outpatient and day care) and hospital palliative care to support to patients with advanced, life shortening illnesses in South Wales. The post offers excellent opportunities to gain experience in all aspects of palliative medicine.

About us

The department is part of the Cancer Services structure, the service is increasingly engaged in the care of people with diagnoses other than cancer dealing with a wide range of life limiting illnesses.

The Specialist Palliative Care Service within SB University Health Board is provided within:

T Olwen which is located in the grounds of Morriston Hospital. The Specialist Inpatient Unit has 10-14 inpatient beds.

The Community Palliative Care Team is a consultant-led multidisciplinary team. The multi-professional team consists of doctors, nurses, physiotherapy and occupational therapy and visits patients at home according to need. Alternatively, when appropriate, patients support is provided through Outpatient clinics, which are held in Y Rhosyn (Neath Port Talbot Hospital) and are part of the Community Palliative Care support.

The Hospital Palliative Care Team works across the two major hospital sites for Swansea (Morriston and Singleton) along with Cefn Coed Hospital (Mental Health) and Gorseinon Hospital (community hospital). The Neath Port Talbot community team cover Neath Hospital inpatients. It is a Consultant-led service and has a full time Specialist Registrar; an Associate Specialist Doctor and a specialty Doctor. There is a team of Clinical Nurse Specialists.

Job description

Job responsibilities

The post holder will be required to:

  • Be a core member of the Specialist Palliative Care MDTs
  • Be a core member of the relevant clinical service groups.
  • Undertake all work in accordance with the Health Board Trusts procedures and operating policies;
  • Participate in job appraisal and review.

You will be expected to deputise for absent colleagues and in exceptional, unforeseen circumstances may have to perform additional duties to ensure the safe running of the department. Where such duties take place outside your normal contracted duties, you will receive either an equivalent off duty period or will be remunerated appropriately.

You will provide advice to clinicians in hospital and primary care, originating either from telephone calls or ward referrals.

The appointee will be required to maintain his/her private residence in contact with the public telephone service.

Travelling between Hospital/Clinic sites may be required to undertake duties. In exceptional circumstances it may also be necessary to undertake duties at other locations with the Health Board.

Together with colleagues, be responsible for the provision of specialist palliative care to the SB University Health Board and for the community it serves 24/7

Primarily to provide Palliative Medicine consultations as part of the Community Palliative care advisory team, under consultant supervision or as part of the Hospital Palliative care team under consultant supervision.

Ensure availability and accessibility.

Within the level of their competency, provide telephone advice on specialist palliative care to primary and secondary care teams for the SBUHB population in and out of hours (in liaison with the consultant).

Work with colleagues in primary and secondary care to provide effective symptom control, appropriate rehabilitation and terminal care for patients within the hospital and community.

Have continuing responsibility for the care of patients under his or her care, allowing for proper delegation to, and training of, staff.

Have responsibility for the proper functioning of the department in line with the operational policies of the Health Board.

Provide middle grade non-residential on call cover in association with other colleagues to support health care professionals in any setting and 7/7 CNS working.

Have direct responsibility for patients in the specialist inpatient unit- T olwen (first on call) out of hours.

The Consultant rota currently includes Specialist Palliative Care cover in any setting.

There are specialist beds in T Bryngwyn (six beds), Prince Phillip Hospital, Hywel Dda Health Board, and 10-14 beds in T Olwen. 3.1 WTE Consultant in Palliative Medicine Hywel Dda participates in this on-call rota currently.
