Job summary
As a post-registration foundation training (PRFT) pharmacist within the renal pharmacy team in Swansea you will be responsible for delivering a specialist clinical pharmacy service to nominated wards and renal outpatient areas in SBU and satellite units in Hywel Dda and Cwm Taf Health Boards.
Through the PRFT Programme you will be provided with a dedicated practice supervisor, a Designated Prescribing Practitioner (DPP) and protected learning time to achieve your goals; including an Independent Prescribing (IP) award should the IP module be undertaken in the second year of the programme.
As a clinical pharmacist you will provide specialist advice and care for people with kidney disease. Should you undertake the IP module in year 2, successfully complete the module and an annotation to the GPhC register has been made, you may be qualified to prescribe towards the end of this programme.

Main duties of the job
Deliver a specialist clinical pharmacy service to nominated wards and renal outpatient areas in SBU and satellite units in Hywel Dda and Cwm Taf Health Boards.
Complete the Health Education and Improvement Wales (HEIW) / Cardiff University post-registration foundation training programme.
Undertake a review of inpatient medication charts and dialysis Electronic Prescribing andMedicines Administration (EPMA) charts.
Counsel patients on the safe and effective use of medicines to ensure maximum benefit.
Supervise work carried out by other pharmacy staff when required, ensuring compliance with agreed pharmaceutical standards.
Ensure storage of medicines on Wards, Dialysis Units and in the Pharmacy Department complies with national legislation and local policies.
Liaise with the renal pharmacists and technicians, communicating any pharmaceutical problems encountered.
Attend and participate in departmental educational and business meetings.
Complete competency based learning objectives.
Identify and lead drug related audit projects.
Provide advice to senior renal pharmacists for service development.
Ensure adherence to national and local protocols and guidelines.
Monitor drug use and expenditure and provide reports to senior renal pharmacists.
Maintain accurate and up to date medication records in the central renal database.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
